基于“脾气散精”理论探讨从脾论治冠心病调节血脂水平效果研究

摘　　要：冠心病是全球范围内严重威胁公共卫生健康的慢性病,随着社会经济的发展以及国民生活方式的变化,冠心病的发病率仍持续增高。血脂异常是冠心病的重要危险因素之一,2019年《中国心血管健康与疾病报告》发布,显示现阶段我国成年人对于血脂异常的知晓率、治疗率以及控制率总体仍处于较低的水平,对于冠心病患者的降脂治疗率、达标率现状堪忧。中医药的治疗具有多靶点、多环节、多途径的优势,研究团队通过既往研究已证实了从脾论治冠心病可调节血脂水平、改善脂质沉积、延缓动脉粥样硬化的发生与发展,故文章以《黄帝内经》"脾气散精"理论为指导,以中医药"治未病"理念为启示,总结和分析从脾论治冠心病患者调节血脂的治疗效果,探讨从脾论治动脉粥样硬化性疾病的作用机制,认为"脾虚生痰"所致冠心病,为脾虚失健、脾不散精、痰浊内生、因痰致瘀、痰瘀互结,治以"益气健脾"治其本,"祛痰化浊"治其标,适当配以活血化瘀药,在调控血脂水平方面疗效确切。为中医"从脾论治"冠心病提供科学依据,丰富脾脏象理论。Coronary heart disease is a chronic disease that seriously threatens public health in the world.With the development of society and economy and the change of people’s lifestyle,the incidence of coronary heart disease continues to increase.Dyslipidemia is one of the important risk factors for coronary heart disease.In 2019,the China Cardiovascular Health and Disease Report was released,showing that the awareness rate,treatment rate and control rate of dyslipidemia among Chinese adults are still at a low level at this stage.The current status of the rate of lipid-lowering treatment and the rate of compliance for patients with coronary heart disease is worrying.The treatment of traditional Chinese medicine has the advantages of multiple targets,multiple links,and multiple pathways.Through previous studies,the research team has confirmed that the treatment of coronary heart disease from the spleen can regulate blood lipid levels,improve lipid deposition,and delay the occurrence and development of atherosclerosis.Therefore,this article is guided by the theory of“spleen-Qi transporting the essence to the whole body”in Inner Canon of Huangdi,and inspired by the concept of“preventing disease”in traditional Chinese medicine.It summarizes and analyzes the therapeutic effects of regulating blood lipids in patients with coronary heart disease based on the spleen theory,and explores the mechanism of action in the treatment of atherosclerotic diseases based on the spleen theory.It is believed that coronary heart disease caused by“spleen deficiency produces phlegm”is caused by spleen deficiency,spleen Qi can not distribute essence,turbid phlegm,blood stasis caused by phlegm,and mutual accumulation of phlegm and blood stasis.In the treatment,the roots are treated by“invigorating the Qi and spleen”,and the symptoms are treated by“expecting phlegm and turbidity”.The medicines for promoting blood circulation and removing blood stasis are appropriately matched to have a definite effect in regulating blood lipid
