益气养心汤对冠心病室性早搏患者(气虚血瘀型)左心室功能、外周血内皮祖细胞及自主神经失衡影响

摘　　要：目的探讨益气养心汤对冠心病室性早搏患者(气虚血瘀型)的左心室功能、外周血内皮祖细胞及自主神经失衡的影响。方法选择2019年1月—2020年6月就诊的108例冠心病室性心律失常患者进行前瞻性研究,根据随机数字表法将纳入研究的病例分为联合中药组和对照组,两组患者行不同治疗方案。对照组行常规扩张冠状动脉、调节血压、抗凝、调脂等基础治疗方案,联合中药组在此基础上服用益气养心汤,疗程均为8周,疗程结束后统计疗效。对比两组患者治疗前后中医证候积分、左心室功能、血清一氧化氮(NO)含量、血栓素(TXB_(2))含量、内皮素(ET-1)含量以及正常窦性心搏间期标准差(SDNN)、全程相邻窦性心搏间期之差的均方根(RMSSD)以及低频功率(LF)和高频功率(HF)。结果治疗后,两组患者各项中医证候积分均下降(P<0.05),且联合中药组明显低于对照组(P<0.05);联合中药组疗效分级情况明显高于对照组(P<0.05);治疗后,两组患者LVEF均升高(P<0.05),且联合中药组明显高于对照组(P<0.05);两组LVEDD和LVESD均降低(P<0.05),且联合中药组明显低于对照组(P<0.05);治疗后,两组血清NO均升高(P<0.05),且联合中药组明显高于对照组(P<0.05);两组TXB_(2)和ET-1均降低(P<0.05),且联合中药组明显低于对照组(P<0.05);治疗后,两组SDNN、RMSSD、LF、HF均升高(P<0.05),且联合中药组明显高于对照组(P<0.05)。结论益气养心汤对冠心病心室早搏患者疗效较好,可有效增强患者左心室功能,促进内皮细胞功能并改善自主神经失衡,促进患者康复。Objective To explore the effects of Yiqi Yangxin Decoction(益气养心汤)on left ventricular function,peripheral blood endothelial cells and autonomic nerve imbalance in patients with coronary heart disease with ventricular premature beats(Qi deficiency and blood stasis type).Methods 108 patients with coronary heart disease and ventricular arrhythmia who were admitted to our hospital from January 2019 to June 2020 were selected for prospective study.According to the random number table method,the included cases were divided into a combined Chinese medicine group and a control group.Group patients underwent different treatment plans.The control group received routine coronary dilation,blood pressure regulation,anticoagulation,lipid regulation and other basic treatment programs.The combined Chinese medicine group took Yiqi Yangxin Decoction on this basis.The course of treatment was 8 weeks.The curative effect was calculated after the treatment.Compared the TCM symptom scores,left ventricular function,serum nitric oxide(NO)content,thromboxane(TXB_(2))content,endothelin(ET-1)content,and standard deviation of normal sinus heartbeat interval(SDNN),root mean square(RMSSD)and low frequency power(LF)and high frequency power(HF)of the difference between adjacent sinus heart beats throughout the whole process.Results After treatment,the scores of various TCM syndromes of the two groups of patients decreased(P<0.05),and the combined Chinese medicine group was significantly lower(P<0.05);the combined Chinese medicine group was significantly higher(P<0.05);After treatment,the LVEF of both groups increased(P<0.05),and the combined Chinese medicine group was significantly higher(P<0.05);both groups of LVEDD and LVESD decreased(P<0.05),and the combined Chinese medicine group significantly lower(P<0.05);after treatment,both groups of serum NO increased(P<0.05),and the combined Chinese medicine group was significantly higher(P<0.05);both TXB_(2) and ET-1 decrease(P<0.05),and the combined traditional Chinese medicine group was sig
