参芪瓜蒌薤白半夏汤在药物球囊干预不稳定型心绞痛单支罪犯血管中的应用

摘　　要：目的探讨参芪瓜蒌薤白半夏汤在药物球囊干预不稳定型心绞痛(unstable angina pectoris,UAP)单支罪犯血管中的应用。方法回顾性分析2018年1月—2020年9月来医院心内科就诊的不稳定型心绞痛行经皮冠状动脉成形术(percutaneous transluminal coronary angioplasty,PTCA)的患者,分为单纯药物球囊干预(对照组)和瓜蒌薤白半夏汤联合药物球囊干预(观察组),每组各33例,记录门诊随访3个月内主要心血管事件(MACE)发生,观察两组患者治疗前后中医证候评分,测定治疗前后血脂水平,评估瓜蒌薤白半夏汤的安全性。结果随访期间两组患者MACE事件差异无统计学意义(P>0.05);观察组中医证候积分改善优于对照组(P<0.05),能够明显改善患者甘油三酯(TC)、低密度脂蛋白(LDL)水平,差异具有统计学意义(P<0.05);观察组NO水平明显高于对照组(P<0.05),观察组Hcy水平明显低于对照组(P<0.05),治疗后观察组各项安全性指标未见明显异常。结论参芪瓜蒌薤白半夏汤联合药物球囊干预UAP单支血管病变效果临床疗效确切,参芪瓜蒌薤白半夏汤能够明显改善患者的生活质量,能够改善内皮功能,进一步降低LDL及TC,延缓术后动脉硬化形成进程。Objective To explore the application of Shenqi Gualou Xiebai Banxia Decoction(参芪瓜蒌薤白半夏汤)in drug balloon intervention of single criminal vessel in unstable angina pectoris(UAP).Methods The patients with unstable angina pectoris treated with single vessel percutaneous transluminal coronary angioplasty(PTCA)in the department of cardiology of our hospital from January 2018 to September 2020 were retrospectively analyzed.They were divided into simple drug balloon intervention(control group)and Gualou Xiebai Banxia Decoction combined with drug balloon intervention(observation group),with 33 cases in each group.The occurrence of major cardiovascular events(MACE)within 3 months of outpatient follow-up was recorded.The TCM symptom score before and after treatment,the blood lipid level before and after treatment and the safety of Gualou Xiebai Banxia Decoction were observed.Results There was no significant difference in mace events between the two groups during followup(P>0.05);The improvement of TCM symptom score in the observation group was better than that in the control group(P<0.05),which could significantly improve the levels of TC and LDL(P<0.05);The level of no in the observation group was significantly higher than that in the control group(P<0.05),and the level of Hcy in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Shenqi Gualou Xiebai Banxia Decoction combined with drug balloon has a definite clinical effect on UAP single vessel disease.Shenqi Gualou Xiebai Banxia Decoction can significantly improve the quality of life,improve endothelial function,further reduce LDL and TC,and delay the formation process of postoperative arteriosclerosis.
