摘  要:目的探讨肺康益气颗粒对脂多糖(LPS)诱导的大鼠急性肺损伤(ALI)的保护作用及免疫调节作用,为新型冠状病毒肺炎(COVID-19)恢复期的临床治疗提供理论依据。方法采用气管内滴注LPS(5 mg/kg)构建ALI模型,将大鼠随机分为正常组,假手术组,模型组,肺康益气颗粒低(2.7 g/kg)、中(5.4 g/kg)、高(10.8 g/kg)剂量组,六君子丸组(1.62 g/kg)和地塞米松片组(1.08 mg/kg)。给药7 d后处死动物,摘取胸腺、脾、肺组织称重,计算脏器指数及肺组织湿/干重比,HE染色观察肺组织病理改变,ELISA法测定肺组织白细胞介素-1β(IL-1β)、前列腺素E2(PGE2)含量。采用腹腔注射环磷酰胺(50 mg/kg)法构建免疫功能低下模型,观察小鼠网状内皮系统吞噬能力。结果与模型组比较,肺康益气颗粒低剂量组脾指数、胸腺指数显著降低(P<0.05);肺康益气颗粒低、高剂量组左肺湿重、肺组织湿/干重比均显著降低(P<0.05或P<0.01);肺康益气颗粒中、高剂量组病理评分均显著降低(P<0.05或P<0.01);肺康益气颗粒低、中、高剂量组肺组织中IL-1β、PGE2含量显著降低(P<0.05或P<0.01);肺康益气颗粒中、高剂量组小鼠吞噬指数显著降低(P<0.05或P<0.01)。结论肺康益气颗粒能明显减轻LPS诱导的ALI大鼠肺水肿,缓解肺组织损伤及炎症反应,对急性肺损伤有一定的保护作用,且能提高机体的免疫功能,适用于COVID-19肺炎恢复期肺脾气虚证的治疗。Objective To explore the protective effect of Feikang Yiqi Granule(肺康益气颗粒)on lipopolysaccharide(LPS)-induced acute lung injury(ALI)in rats and its immunomodulatory effect,and to provide theoretical basis for the treatment of convalescent patients with corona virus disease 2019(COVID-19).Methods The ALI model was established by intratracheal instillation of LPS(5 mg/kg),and the rats were randomly divided into normal group,sham operation group,model group,low dose(2.7 g/kg),medium dose(5.4 g/kg),and high dose(10.8 g/kg)groups of Feikang Yiqi Granule,Liujunzi Pills(六君子丸,1.62 g/kg)group and dexamethasone tablets(1.08 mg/kg)group.After 7 days of administration,the thymus,spleen and lung tissues were weighed,and the organ index and lung tissue wet/dry weight ratio were calculated.The pathological changes of lung tissue were observed by HE staining,and the content of IL-1βand PGE2 in lung tissue was determined by ELISA.The immunosuppression model was established by intraperitoneal injection of cyclophosphamide(50 mg/kg),in order to observe the phagocytic ability of the mouse reticuloendothelial system.Results Compared with the model group,the spleen index and thymus index of the low-dose Feikang Yiqi Granule group were significantly reduced(P<0.05);the wet weight of the left lung and the wet/dry ratio of lung tissue were significantly decreased in the low and high-dose groups(P<0.05 or P<0.01);the pathological scores of the medium and high dose groups were significantly decreased(P<0.05 or P<0.01);the levels of IL-1βand PGE2 in the lung tissues of the low,medium and high dose groups were significantly reduced(P<0.05 or P<0.01);the phagocytic index of mice in the middle and high dose groups of Feikang Yiqi Granule was significantly reduced(P<0.05 or P<0.01).Conclusion Feikang Yiqi Granule could significantly reduce the pulmonary edema induced by LPS in ALI rats,relieve the lung tissue injury and inflammatory reaction,had a certain protective effect on acute lung injury,and could improve the immune f
