摘  要:目的:探讨室性心律失常患者血清miR-155、miR-130a表达及IL-10、IL-2的变化与临床意义。方法:2013年8月至2018月2月,选择在北京市垂杨柳医院心内科诊治的急性冠状动脉综合征患者1150例作为研究对象,采用心电图检测室性心律失常情况;采用qRTPCR与ELISA法测定血清miR-155、miR-130a表达及IL-10、IL-2表达情况。结果:在1150例患者中,室性心律失常发生率为13.0%。室性心律失常患者的miR-155、miR-130a相对表达量显著高于非室性心律失常患者(P<0.05)。室性心律失常患者的血清IL-10、IL-2表达水平显著高于非室性心律失常患者(P<0.05),Spearman相关分析显示,心律失常Lown分级与miR-155、miR-130a、IL-10、IL-2呈显著正相关性(P<0.05);Logistic回归分析显示,miR-130a(OR=1.499,95%CI:1.431~1.578,P=0.022)、IL-2(OR=1.497,95%CI:1.017~1.183,P=0.006)为导致急性冠状动脉综合征室性心律失常的主要危险因素(P<0.05)。结论:室性心律失常患者多伴有血清miR-155、miR-130a、IL-10、IL-2高表达,miR-130a与IL-2是导致急性冠状动脉综合征患者发生室性心律失常的重要因素。Objective:To investigate the changes of serum miR-155、miR-130a,and IL-10 and IL-2 in patients with ventricular arrhythmia and its clinical significance.Methods:From August 2013 to February 2018,1150 cases of acute coronary syndrome in the Department of Cardiology of Beijing Chui Yang Liu Hospital were selected as the subjects.The ventricular arrhythmia were detected by electrocardiogram,and the expression of miR-155,miR-130a and the expression of IL-10 and IL-2 were measured by qRT-PCR and ELISA.Results:The ventricular arrhythmia incidence rates were 13%in the 1150 cases.The relative expressions of miR-155,miR-130a in patients with ventricular arrhythmia were significantly higher than those in patients with non ventricular arrhythmia(P<0.05).The levels of serum IL-10 and IL-2 in patients with ventricular arrhythmia were significantly higher than those in patients with non ventricular arrhythmia(P<0.05).The levels of miR-155,mir-130a,IL-10 and IL-2 were positively correlated with the lown grade of arrhythmia(P<0.05).The Spearman correlation analysis showed that the lown classification of the patients with arrhythmia were significant positive correlation with miR-155,miR-130a,IL-10 and IL-2(P<0.05),Logistic regression analysis showed that miR-155(OR=1.499,95%CI:1.431-1.578,P=0.022)and IL-2(OR=1.497,95%CI:1.017-1.183,P=0.006)were the main independent risk factors leading to ventricular arrhythmia in acute coronary syndrome.Conclusions:The high expression of miR-155,miR-130a,and excessive release of IL-10 and IL-2 in patients with ventricular arrhythmia,and miR-130a and IL-2 are the main factors leading to ventricular arrhythmia in patients with acute coronary syndrome.
