摘  要:目的观察葛根素对脱氢表雄酮(DHEA)诱导的多囊卵巢综合症(PCOS)大鼠糖代谢、炎性因子及TLR4/NF-κB信号通路的影响。方法通过皮下注射DHEA建立PCOS大鼠模型,随机分为模型组、二甲双胍组、二甲双胍+葛根素组,每组20只。二甲双胍组灌胃给予二甲双胍粉末(270 mg/kg),二甲双胍+葛根素组灌胃给予二甲双胍粉末(270 mg/kg)和腹腔注射葛根素注射液(100 mg/kg),模型组腹腔注射等容量的生理盐水,连续给药4周,另随机选取20只未造模大鼠作为对照组。治疗结束后记录体质量,检测空腹血糖(FBG)、餐后2小时血糖(2 h PBG)、空腹胰岛素(FINS)水平,并计算胰岛素抵抗指数值(IR)。ELISA检测大鼠血清中大鼠血清中促黄体生成素(LH)、睾酮(T)、卵泡刺激素(FSH)、白细胞介素6(IL-6)、C反应蛋白(CRP)和肿瘤坏死因子(TNF-α)水平,RT-qPCR检测TLR4、NF-κB、ox-LDL mRNA表达。结果与对照组比较,模型组大鼠体质量,血清LH、T、FBG、2 h PBG、FINS、IR、IL-6、TNF-α、CRP水平及卵巢TLR4、NF-κB、ox-LDL表达均升高(P<0.05),血清FSH水平降低(P<0.05);与模型组比较,二甲双胍组和二甲双胍+葛根素组大鼠体质量,血清LH、T、FBG、2 h PBG、FINS、IR、IL-6、TNF-α、CRP水平及卵巢TLR4、NF-κB、ox-LDL表达均降低(P<0.05),血清FSH水平升高(P<0.05),卵巢囊泡数目减少(P<0.05),颗粒细胞层厚度增加(P<0.05),且二甲双胍+葛根素组作用更显著(P<0.05)。结论葛根素和二甲双胍联用可能通过调控TLR4/NF-κB通路降低PCOS大鼠体质量、胰岛素抵抗及血糖、性激素、炎性因子、ox-LDL水平。AIM To investigate the effects of puerarin on glycometabolism and TLR4/NF-κB signaling pathway in rats with polycystic ovary syndrome(PCOS) induced by dehydroepiandrosterone(DHEA). METHODS PCOS rats were established by subcutaneous injection of DHEA and randomly divided into model group for 4-week intraperitoneal administration of normal saline, metformin group for 4-week metformin powder use(270 mg/kg), and the trial group for 4-week both metformin powder use(270 mg/kg) and intraperitoneal injection of puerarin(100 mg/kg), with 20 rats in each group. Another 20 intact rats were randomly selected into the control group. After the treatment, the rats had their body weight recorded, and their levels of FBG, 2 h PBG and FINS detected for IR calculation. The determinations of serum levels of LH, T, FSH, IL-6, CRP and TNF-α by ELISA;and mRNA expressions of TLR4, NF-κB and ox-LDL by RT-qPCR were performed as well. RESULTS Compared with the control group, the model group shared increase in body weight, serum levels of LH, T, FBG, 2 h PBG, FINS, IR, IL-6, TNF-α and CRP, and expressions of TLR4, NF-κB and ox-LDL in ovary(P<0.05), and decrease in serum FSH level(P<0.05). Compared with the model group, groups intervened with either metformin or combination therapy of metformin and puerarin were observed with decrease in body weight, serum levels of LH, T, FBG, 2 h PBG, FINS, IR, IL-6, TNF-α and CRP, and the expressions of TLR4, NF-κB and ox-LDL in ovary(P<0.05), increase in serum FSH level(P<0.05), decrease in number of ovarian vesicles(P<0.05), and increase in thickness of granulosa cell layer(P<0.05);and the trial group demonstrated an even more significant efficacy(P<0.05). CONCLUSION The combination therapy of puerarin and metformin upon PCOS rats may reduce their body weight, insulin resistance, and the levels of blood glucose, sex hormones, inflammatory factors and ox-LDL via regulation of TLR4/NF-κB pathway.
