复方黄柏液涂剂超声引导下置管冲洗对内痈患者的临床疗效

摘　　要：目的考察复方黄柏液涂剂超声引导下置管冲洗对内痈患者的临床疗效。方法84例患者随机分为对照组和观察组,每组42例,在常规治疗(补液、全身抗感染、器官功能维护等)基础上,对照组采用庆大霉素超声引导下置管冲洗,观察组采用复方黄柏液涂剂超声引导下置管冲洗。检测临床疗效、穿刺细菌培养阳性率、炎症反应指标(WBC、NEU、CRP)、组织生长和愈合指标(VEGF、EGF、bFGF)、脓腔体积、闭合时间、住院时间、复发率变化。结果观察组总有效率高于对照组(P<0.05),住院时间更短(P<0.05)。治疗后第3天,观察组WBC、NEU、CRP低于对照组(P<0.05),第5天观察组NEU、CRP更低(P<0.05)。治疗后第3、5、7天,观察组VEGF、EGF、bFGF高于对照组(P<0.05)。治疗后第3、7、14天,2组脓腔体积缩小(P<0.05),第14天观察组更明显(P<0.05)。2组细菌培养阳性率、随访6个月复发率比较,差异无统计学意义(P>0.05),并且随访12个月2组无复发。结论复方黄柏液涂剂超声引导下置管冲洗对内痈患者具有较好的临床疗效。AIM To investigate the clinical effects of ultrasound-guided catheter lavage with Compound Huangbai Liquid on patients with internal carbuncle. METHODS Eighty-four patients were randomly assigned into control group(42 cases) for intervention of both ultrasound-guided catheter lavage with gentamicin and conventional treatment(fluid infusion, systemic anti-infection, organ function maintenance, etc.), and observation group(42 cases) for intervention of both ultrasound-guided catheter lavage with Compound Huangbai Liquid and conventional treatment. The changes in clinical efficacy, positive rate of bacterial culture after puncture, inflammatory response indices(WBC, NEU, CRP), tissue growth and healing indices(VEGF, EGF, bFGF), abscess cavity volume, closure time, hospital stay and recurrence rate were detected. RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05), along with shorter hospital stay(P<0.05). On the third day after the treatment, the observation group displayed lower WBC, NEU and CRP than the control group(P<0.05), and lower NEU and CRP were observable in the observation group on the fifth day(P<0.05). On the third, fifth and seventh days after the treatment, the observation group displayed higher VEGF, EGF and bFGF than the control group(P<0.05). On the third, seventh and fourteenth days after the treatment, the two groups displayed decreased abscess cavity volume(P<0.05), especially for the observation group on the fourteenth days(P<0.05).No significant differences in positive rate of bacterial culture after puncture and recurrence rate at 6-month follow-up were found between the two groups(P>0.05), and no recurrence occured in the two groups at 12-month follow-up. CONCLUSION For the patients with internal carbuncle, ultrasound-guided catheter lavage with Compound Huangbai Liquid exhibits good clinical efficacy.
