犬高度房室传导阻滞合并库兴氏综合征的病例报告

摘　　要：近半年来每月会有一次晕厥且腹围逐渐增大的银狐犬,多所动物医院就诊治疗未有效果,通过对患病犬进行病史调查、一般临床检查、血常规检查、X线检查、超声心动图检查、心电图检查确诊为高度房室传导阻滞合并库兴氏综合征。采用手术方法进行人工节律器植入术,并在术后采取消炎镇定措施防止导线移位,同时服用保肝利胆药以及类固醇酶抑制剂治疗库兴氏综合征。至术后70 d时,患犬整体预后良好。诊疗过程表明,高度房室传导阻滞与库兴氏征的发生并无明确因果关系,应为偶然同时发生,在治疗时可以先对危及生命的高度房室传导阻滞进行手术。In recent half a year,one Spitzhad experienced syncope once a month and the abdominal perimeter gradually increased.After treatment in many hospitals,the dog was not healed.Through history investigation,general clinical examination,blood examination,X-ray examination,echocardiography and electrocardiogram examination,the dog was diagnosed as comorbidity with advanced atrioventricular block and Cushing’s disease.Artificial pacemaker implantation was performed surgically,and anti-inflammatory and sedation measures were taken to prevent wire displacement.Hepatoprotective and cholagogic drugs and steroid enzyme inhibitors were also taken to treat Cushing’s syndrome.The overall prognosis of the dogs was good at 70days after operation.The diagnosis and treatment process suggested that there was no clear causal relationship between advanced atrioventricular block and Cushing’s disease.It should be coincidental,and surgery for the life-threatening high atrioventricular block could be performed firstly.
