摘  要:用弹力蛋白酶直接破坏犬腹主动脉管壁中膜的弹力板结构,快速构建一种适用于腔内治疗研究的犬腹主动脉瘤疾病模型。手术分离犬腹主动脉2 cm左右,将5 mL弹力蛋白酶(100 U/mL)注射到犬腹主动脉管壁中膜,导致犬腹主动脉管壁中膜结构和负荷失衡,不能抵抗管腔内血流压力而发生阶段性扩张。超声每天观察并记录犬腹主动脉瘤样扩张情况。血常规分析记录术后每天犬炎性细胞变化,病理组织学观察犬腹主动脉壁的组织结构、炎性细胞浸润及弹力纤维的损伤程度。结果显示,腹主动脉管壁中膜灌注蛋白酶的犬均在术后8 d形成瘤样扩张,与对照组比较差异显著(P<0.05);超声检查发现犬腹主动脉扩张存在快速扩张期(1 d~3 d)和逐渐扩张期(4 d~8 d);术后1 d~2 d各组白细胞总数显著上升、淋巴细胞数量无明显变化,术后3 d~8 d对照组各项指标均恢复至术前水平,试验组中性粒细胞数量升高,与术前差异不显著;病理组织学观察表明,主动脉管壁中膜和外膜炎性细胞浸润,弹力纤维断裂、降解,细胞外基质受损。结果表明通过此方法制作的犬腹主动脉瘤疾病模型具有成瘤时间短、操作简单、病理学特征相似和渐进性扩张的特点,适用于腔内治疗研究。Rapid construction of a disease model for canine abdominal aortic aneurysms suitable for intracavitary therapeutic studies using elastin to directly disrupt the elastin structure of the tunica media in the wall of the canine abdominal aorta.Surgical separation of the canine abdominal aorta around 2 cm from the abdominal aorta,injection of 5 ml elastase(100 U/mL)into the tunica media of the canine abdominal aortic wall resulted in imbalance between the tunica media structure and load,failure to resist intraluminal blood flow pressure and occurrence of stage dilation.The aneurysmal dilatation of the abdominal aorta of the dogs was observed and recorded daily by ultrasound.Changes of inflammatory cells of dogs were recorded every day after surgery by blood routine analysis.Histological examination of the wall of the canine abdominal aorta was performed to evaluate the tissue structure,inflammatory cell infiltration,and damage to elastic fibers.The results showed that all dogs infused with protease in the tunica media of the abdominal aortic canal developed aneurysmal dilatation 8 days after surgery,which was significantly different from the normal control group(P<0.05);Ultrasonography revealed that there were 2 plateau phases in the process of canine abdominal aortic dilatation,namely the rapid dilation phase(1-3 days)and the gradual dilation phase(4-8 days);Blood routine examination showed that,the total number of white blood cells in each group increased significantly,and lymphocytes did not change significantly on the 1-2 days after surgery,all indexes in the control group returned to their preoperative levels on 3-8 days after surgery,neutrophils in the elastase group increased,not significantly different from the preoperative levels;Histological section results showed infiltration of inflammatory cells in the tunica media and adventitia of the aorta,fragmentation and degradation of elastic fibers,and damage to the extracellular matrix.Our results show that the canine abdominal aortic aneurysm disease model prod
