经尿道分叶式剜除配合电切术治疗良性前列腺增生的效果观察

摘　　要：目的探讨经尿道分叶式剜除配合电切术治疗良性前列腺增生的临床效果。方法将94例良性前列腺增生患者随机分为观察组和对照组各47例。对照组采用经尿道双极等离子电切术治疗,观察组采用经尿道分叶式剜除配合电切术治疗,比较两组的手术相关指标、生活质量及并发症发生率。结果观察组术中出血量少于对照组,手术时间、导管留置时间及膀胱冲洗时间均短于对照组,且前列腺切除质量高于对照组(P<0.05)。治疗前,两组的QOL评分比较无统计差异(F>0.05);治疗后1个月、2个月,观察组的QOL评分均高于对照组(P<0.05)。观察组的并发症发生率为2.13%,低于对照组的17.02%(P<0.05).结论经尿道分叶式剜除配合电切术治疗良性前列腺增生效果显著,可有效改善手术指标,提高患者的生活质量,降低并发症发生率,值得临床推广。Objective To explore the clinical effect of transurethral segmented enucleation combined with electric resection in the treatment of benign prostatic hyperplasia.Methods 94 patients with benign prostatic hyperplasia were randomly divided into observation group and control group,with 47 cases in each group.The control group was treated with transurethral bipolar plasma resection,and the observation group was treated with transurethral segmented enucleation combined with electric resection.The surgical related indicators,quality of life and incidence of complications were compared between the two groups.Results The intraoperative blood loss of the observation group was less than that of the control group,the surgical time,catheter indwelling time and bladder washing time were shorter than those of the control group,and the prostatectomy quality was higher than that of the control group(P<0.05).Before treatment,no statistical difference was found in the QOL score between the two groups(P>0.05).1 month and 2 months after treatment,the QOL scores of the observation group were higher than those of the control group(P<0.05).The incidence of complications in the observation group was 2.13%,lower than 17.02%in the control group(P<0.05).Conclusions Transurethral segmented enucleation combined with electric resection has significant effect in the treatment of benign prostatic hyperplasia,and can effectively improve the surgical indicators,improve the quality of life of patients,and reduce the incidence of complications,which is worthy of clinical promotion.
