摘　　要：目的探讨新生儿缺氧缺血性脑病(HIE)患儿头部亚低温治疗预后不良的影响因素。方法将我院2019年1月至2021年3月行头部亚低温治疗预后良好的48例新生儿HIE患儿纳入预后良好组,另将同期行头部亚低温治疗预后不良的48例新生儿HIE患儿纳入预后不良组。统计所有患儿基线资料,经单因素、多因素分析找出新生儿HIE患儿头部亚低温治疗预后不良的影响因素。结果两组的羊水异常占比、基质金属蛋白酶-9(MMP-9)水平、开始治疗时间比较差异有统计学意义(P<0.05).多元Logistic回归模型分析结果显示,羊水异常、MMP-9水平过高、开始治疗时间延长是新生儿HIE患儿头部亚低温治疗预后不良的危险因素(OR>1,P<0.05)。结论羊水异常、MMP-9水平过高、开始治疗时间延长是新生儿HIE头部亚低温治疗预后不良的影响因素。Objective To explore the influencing factors of prognosis in infants with neonatal hypoxic ischemic encephalopathy(HIE).Methods 48 HIE neonates with good prognosis of head mild hypothermia treatment from January 2019 to March 2021 in our hospital were selected as good prognosis group,and 48 HIE neonates with poor prognosis of head mild hypothermia treatment at the same time were selected as poor prognosis group.The baseline data of all children were counted,and the influencing factors for poor prognosis of head mild hypothermia treatment in HIE neonates were found through univariate and multivariate analysis.Results Statistical difference was found in the proportion of abnormal amniotic fluid,matrix metalloproteinase-9(MMP-9)level,and treatment start time between the two groups(P<0.05).Multivariate Logistic regression model analysis results showed that abnormal amniotic fluid,excessive MMP-9 level and prolonged treatment start time are the risk factors for poor prognosis of head mild hypothermia treatment in HIE neonates(OR>1,P<0.05).Conclusions Abnormal amniotic fluid,excessive MMP-9 level,and prolonged treatment start time are the influencing factors for poor prognosis of head mild hypothermia treatment in HIE neonates.
