中药离子导入治疗慢性阻塞性肺疾病急性加重期痰热郁肺证临床观察

摘　　要：目的观察中药离子导入技术辅助治疗慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)痰热郁肺证患者的临床疗效。方法将60例AECOPD痰热郁肺证患者随机分为对照组及观察组,每组30例。两组均予以西医常规治疗,观察组在此基础上增加中药离子导入治疗,10 d为1个疗程,采用圣乔治呼吸问卷(St.George's Respiratory Questionnaire,SGRQ)评价患者症状改善情况;采用股动脉二氧化碳分压(partial pressure of carbon dioxide,PaCO_(2))、氧合指数评价患者呼吸通气功能改善情况;采用外周静脉血超敏C反应蛋白(hyper-sensitive C-reactive protein,hs-CRP)、血沉(erythrocyte sedimentation rate,ESR)、血清降钙素原(procalcitonin,PCT)评价血炎症指标改善情况;比较两组患者治疗后的临床疗效。结果与治疗前比较,两组患者治疗后SGRQ得分均显著降低(P<0.05),观察组较对照组得分降低更显著(P<0.05);两组患者治疗后PaCO_(2)均显著降低(P<0.05),氧合指数显著升高(P<0.05),观察组较对照组改善更明显(P<0.05);两组患者治疗后hs-CRP、ESR、PCT水平均显著降低(P<0.05),观察组降低水平更显著(P<0.05);观察组患者疗效优于对照组(P<0.05)。结论中药离子导入技术辅助治疗不仅能改善AECOPD痰热郁肺证患者临床症状,提高其生命质量,还能有效改善其呼吸通气功能,减轻炎症反应。Objective To investigate the clinical effect of traditional Chinese medicine(TCM)iontophoresis as an adjuvant therapy for acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with the syndrome of phlegm-heat obstructing the lungs.Methods A total of 60 patients with AECOPD were randomly divided into control group and observation group,with 30 patients in each group.The patients in the control group were given conventional Western medicine treatment,and those in the observation group were given TCM iontophoresis in addition to the treatment in the control group;each course of treatment was 10 days.St.George's Respiratory Questionnaire(SGRQ)was used to evaluate the improvement in symptoms;arterial partial pressure of carbon dioxide(PaCO_(2))of the femoral artery and oxygenation index were used to evaluate the improvement in respiration ventilation function;high-sensitivity C-reactive protein(hs-CRP),erythrocyte sedimentation rate(ESR),and serum procalcitonin(PCT)were used to evaluate the improvement in inflammatory markers;clinical outcome was compared between the two groups.Results After treatment,both groups had a significant reduction in SGRQ score(P<0.05),and the observation group had a significantly greater reduction than the control group(P<0.05).After treatment,both groups had a significant reduction in PaCO_(2)(P<0.05)and a significant increase in oxygenation index,and the observation group had significantly greater improvements than the control group(P<0.05),After treatment,both groups had significant reductions in the levels of hs-CRP,ESR,and PCT(P<0.05),and the observation group had significantly greater reductions than the control group(P<0.05).The observation group had a significantly better clinical outcome than the control group(P<0.05).Conclusion As an adjuvant therapy for AECOPD with the syndrome of phlegm-heat obstructing the lungs,TCM iontophoresis can improve clinical symptoms,quality of life,and respiration ventilation function,alleviate inflammatory response.
