中国老年人自杀意念影响因素研究的Meta分析

摘　　要：目的 系统评价中国老年人自杀意念的影响因素。方法 计算机检索中国知网(CNKI)数据库,选择涉及老年人自杀意念影响因素的文献,对纳入的文献进行资料提取,采用Review Manager5.3软件进行Meta分析,确定影响因素。结果 共纳入13篇研究文献,老年人抑郁[OR=14.07,95%CI(8.96,22.11)]、患有慢性病[OR=2.13,95%CI(1.85,2.44)]、经济条件困难[OR=2.17,95%CI(1.42,3.30)]、独居[OR=1.44,95%CI(1.22,1.71)]、无稳定的婚姻[OR=1.49,95%CI(1.23,1.81)]、文化程度低下[OR=1.85,95%CI(1.25,2.73)]、日常活动能力下降[OR=2.76,95%CI(2.33,3.27)]是老年人自杀意念的危险因素(P<0.05)。结论 缓解老年人抑郁症状是可以成为预防老年人自杀意念形成的手段,政府和社会应当重点关注经济条件困难、患有慢性病、独居并且没有稳定婚姻、文化程度或日常生活能力低下的老年人。Objective To systematically evaluate the influencing factors for suicidal ideation among the elderly in China. Methods A computer retrieval of the China National Knowledge Base(CNKI) database was carried out to select the literature on the factors affecting the suicidal ideation of the elderly, and to extract data from the included literature, and then Review Manager5.3 software was used to conduct a Meta-analysis to determine the influencing factors. Results A total of 13 research papers were included. Risk factors for suicidal ideation of the elderly were depression [OR=14.07, 95% CI(8.96, 22.11)], chronic diseases [OR=2.13, 95% CI(1.85, 2.44)], financial difficulties [ OR=2.17, 95% CI(1.42, 3.30)], living alone [OR=1.44, 95% CI(1.22, 1.71)], unstable marriage [OR=1.49, 95% CI(1.23, 1.81)] and poor education [OR=1.85, 95%CI(1.25, 2.73)] and decreased activity of daily life [OR=2.75, 95%CI(2.33, 3.27)](P<0.05). Conclusions Relieving depressive symptoms can be a means to prevent suicidal ideation in the elderly. The government and society should focus on the elderly who have financial difficulties, suffer from chronic diseases, live alone and have no stable marriage, have poor education and decreased activity of daily life.
