摘　　要：目的:组织选择性直肠黏膜环切术(TST)加外痔分段切除、芍倍注射液注射治疗重度环状混合痔的临床效果。方法:选择2019年3月—2021年7月我院收治的80例重度环状混合痔患者,采用随机数字表法分为观察组与对照组,各40例。观察组采用TST+外痔分段切除+芍倍注射液注射治疗;对照组采用吻合器痔上黏膜环切术(PPH)+外痔分段切除治疗。对比两组临床疗效、临床指标及术后复发率、疼痛程度。结果:观察组治疗总有效率为100.00%,略高于对照组的95.00%,但组间对比无明显差异(P>0.05);观察组手术时间、肛周功能恢复时间短于对照组,术后出血量及病情复发率低于对照组(P<0.05);观察组术后1天、术后3天、术后7天VAS评分均低于对照组(P<0.05)。结论:TST+外痔分段切除术+芍倍注射液注射治疗重度环状混合痔的临床效果显著,联合治疗方案对患者术后恢复有促进价值,同时还可降低患者病情复发风险与疼痛程度,值得临床推广。Objective:The clinical effect of tissue selective rectal mucosal circumcision(TST)combined with segmental resection of external hemorrhoids and injection of Shaobei injection in the treatment of severe circular mixed hemorrhoids.Methods:From March 2019 to October 2021,80 patients with severe circular mixed hemorrhoids were selected and divided into observation group and control group by random number table method,with 40 cases in each group.The observation group was treated with TST+external hemorrhoids segmental resection+Shaobei injection;the control group was treated with stapled prolapse and hemorrhoids circumcision(PPH)+external hemorrhoids segmented resection.The clinical efficacy,clinical indicators,postoperative recurrence rate and pain degree were compared between the two groups.Results:The total effective rate of treatment in the observation group was 100.00%,slightly higher than 95.00% in the control group,but there was no significant difference between the groups(P>0.05);the operation time and the recovery time of perianal function in the observation group were shorter than those in the control group.The postoperative bleeding and recurrence rate were lower than those of the control group(P<0.05);the VAS scores of the observation group were lower than those of the control group on the 1st,3rd,and 7th postoperative days(P<0.05).Conclusion:The clinical effect of TST+external hemorrhoids segmental resection+Shaobei injection in the treatment of severe circular mixed hemorrhoids is significant.The combined treatment plan has value in promoting the recovery of patients after surgery,and can also reduce the risk of recurrence and pain in patients.It is worthwhile Clinical promotion.
