血清Lp-PLA2、Hcy和GA表达与CHD患者病情进展的关系及对患者预后的判定

摘　　要：目的分析血清脂蛋白相关磷脂酶A2(Lp-PLA2)、血清同型半胱氨酸(Hcy)和糖化血清白蛋白(GA)表达与冠心病(CHD)患者病情进展的关系及对患者预后预测的价值。方法将我院2020年5月至2020年12月间收治的100例CHD患者作为观察组,同期选择50例心肌炎患者作为对照组。检测受试者血清Lp-PLA2、Hcy和GA表达水平;采用Spearman相关性分析探讨Lp-PLA2、Hcy和GA与患者病情严重程度的关系,采用Logistics回归模型分析Lp-PLA2、Hcy和GA联合预测患者预后质量,绘制ROC曲线并计算各指标预测患者预后质量价值。结果观察组患者血清Lp-PLA2、Hcy和GA均明显高于对照组,且差异具有统计学意义(P<0.05);单支病变组、双支病变组和三支病变组患者血清Lp-PLA2、Hcy和GA水平差异具有统计学意义(P<0.05);患者血清Lp-PLA2、Hcy和GA水平与患者病情呈显著性正相关关系(P<0.05);MACE组患者血清Lp-PLA2、Hcy和GA水平明显高于非MACE组,且差异具有统计学意义(P<0.05);Logistics回归模型结果显示越高水平的Lp-PLA2、Hcy和GA,患者预后质量越差(P<0.05);采用Lp-PLA2、Hcy和GA联合应用预测CHD患者预后质量的AUC为0.988,特异性95.18%,灵敏度94.12%,均明显高于各指标单独应用,且差异存在统计学意义(P<0.05)。结论血清Lp-PLA2、Hcy和GA表达与CHD患者病情进展呈显著正相关关系,且三指标联合应用具有较高的预测患者预后质量的价值。Objective To analyze the relationship between expressions of serum lipoprotein-associated phospholipase A2(Lp-PLA2),homocysteine(Hcy)and glycated albumin(GA)and the progression of coronary heart disease(CHD)patients and its prognostic value.Methods 100 CHD patients admitted to our hospital from May,2020 to December,2020 were enrolled as the observation group,while 50 patients with myocarditis were selected as the control group during the same period.Expression levels of serum Lp-PLA2,Hcy and GA of all subjects were detected.Spearman correlation analysis was applied to explore the relationship between Lp-PLA2,Hcy and GA and the severity of the patient’s condition.Logistics regression model was used to analyze the combined application of Lp-PLA2,Hcy and GA to predict the prognosis quality of patients.ROC curve was drawn to evaluate the value of each index to predict the quality of the patient’s prognosis.Results Serum Lp-PLA2,Hcy and GA of the observation group were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).There were significant differences in serum Lp-PLA2,Hcy,and GA levels among the single-vessel disease group,double-vessel disease group,and three-vessel disease group(P<0.05).Serum levels of Lp-PLA2,Hcy and GA were significantly positively correlated with patients’condition(P<0.05).Serum Lp-PLA2,Hcy and GA levels in the MACE group were significantly higher than those in the non-MACE group(P<0.05).The results of the logistics regression model showed that the higher the level of Lp-PLA2,Hcy and GA,the worse the prognosis quality of patients(P<0.05).The combined application of Lp-PLA2,Hcy and GA to predict the prognostic quality of CHD patients had AUC of 0.988,specificity of 95.18%,and sensitivity of 94.12%,which were significantly higher than those of each individual indicator(P<0.05).Conclusion Expressions of serum Lp-PLA2,Hcy and GA are significantly positively correlated with the progression of CHD patients,and the combined application o
