基于“五脏六腑皆令人咳”理论探讨《幼幼集成》论治小儿咳嗽的特色

摘　　要：《幼幼集成》是集清代以前学术成就的一部儿科专著,陈复正对儿科疾病的治疗有许多独到见解,文章通过对其卷三《咳嗽证治》篇的学习,分析陈复正从五脏角度论治小儿咳嗽的辨证思路及选方用药,确立了以人参败毒散为咳门第一方,清肺饮、葶苈丸治“肺咳”,清宁散、集成沆瀣丹治“心咳”方,橘皮汤治“脾咳”,集成金粟丹治“肝咳”,六味地黄汤加麦冬、五味子治“肾咳”,并形成了独具特色的“五脏论咳”的辨治理论,以期师古而不泥古,拓宽临证思路,提高临床疗效。Youyou Jicheng is a pediatrics monographs that collected the academic achievements before the Qing dynasty,there are many special view points and experiences of pediatrics in this book. This article tries to analyze CHEN Fu-zheng’s syndrome differentiation and prescriptions based on the five internal organs on the treatment of children’s cough through the study of the chapter Cough Syndrome and Treatment in Volume 3. It is established that Renshen Baidu Powder is the first choice for cough,Qingfei Yin and Tingli Pills are used to treat ‘lung cough’, Qingning Powder and Integrating Hangxie Dan are used to treat ‘heart cough’, Jupi Decoction is used to treat ‘spleen cough’, Integrating Jinsudan is used to treat ‘liver cough’, Liuwei Dihuang Decoction plus Maidong and Wuweizi is used to treat ‘kidney cough’. Formed a unique diagnosis and treatment theory of ‘five viscera on cough’, in order to learn from the past and not to muddle the past, broaden the clinical thinking and improve the clinical efficacy.
