从标本缓急论治强直性脊柱炎

摘　　要：强直性脊柱炎以本虚标实为特点,肾虚督亏为其本,风寒湿邪或湿热之邪痹阻经络、脉络瘀阻为其标,姜泉教授将本病分为病情活动期与缓解期,采用“急则治标,缓则治本”的治疗原则。病情活动期的治疗当祛邪通络为要:辨证祛邪,急则治标,活血化瘀,通络止痛;缓解期的治疗当补肾活血为要:补肾强督,缓则治本,活血通络,兼顾祛邪。不论急缓,全程注重舒筋活络、顾护脾胃。Ankylosing spondylitis(AS) is characterized by difficiency in origin and excess in superficiality, the deficiency of the kidney and governor vessel is the original reason, the wind-cold-damp evil or damp-heat evil obstruct the meridians and the choroid stasis is the superficial reason. Professor JIANG Quan divides the disease into acute active stage and remission stage, and adopts the treatment principle of ‘symptomatic treatment in acute condition, radical treatment in chronic case’. The main treatment of the acute active stage should be to eliminate evils and dredge collaterals: Eliminating evils based on syndrome differentiation, symptomatic treatment in acute condition;Promoting blood circulation for removing blood stasis, and dredging collaterals for relieving pain. The main treatment of remission is to tonify the kidney and promote blood circulation:tonifying the kidney and strengthening the governor, radical treatment in chronic case, promoting blood circulation for dredging collaterals, co-operating to eliminate evils. No matter in acute active stage or remission, the whole process pay attention to relaxing tendons and activating collaterals, and nourishing the spleen and stomach.
