康氏导水茯苓汤治疗酒精性肝硬化失代偿期水湿内阻证的临床观察

摘　　要：目的:观察康氏导水茯苓汤治疗酒精性肝硬化失代偿期水湿内阻证患者的临床疗效。方法:将70例酒精性肝硬化失代偿期水湿内阻证患者随机分为治疗组与对照组,每组35例,对照组患者采用戒酒及常规对症治疗24周;治疗组患者在对照组治疗的基础上加用康氏导水茯苓汤治疗24周。观察两组患者的中医证候评分、肝功能、血常规、凝血功能指标、Child-Pugh评分、腹围、体质量及24 h尿量变化情况。结果:治疗组与对照组患者临床总有效率分别为81.8%(27/33)与58.1%(18/31),治疗组显著高于对照组(P<0.05);治疗组患者Child-Pugh评分显著低于对照组评分(P<0.05);腹水改善情况、总胆红素降低、腹围缩小、体质量减轻、尿量增加均显著优于对照组(P<0.05)。结论:康氏导水茯苓汤治疗酒精性肝硬化失代偿期水湿内阻证患者的疗效确切,其机制可能与减轻肝脏炎症和改善肝细胞代谢有关。Objective: To observe the clinical efficacy of Kangshi Daoshui Fuling Decoction in treating patients with water dampness stagnation type alcoholic cirrhosis during decompensation. Methods: Seventy patients with water dampness stagnation type decompensated alcoholic cirrhosis were randomly divided into a treatment group and a control group, with 35 cases in each group. Patients in the control group were treated with alcohol withdrawal and conventional symptomatic treatment for 24 weeks. The patients in the treatment group were treated with Kangshi Daoshui Fuling Decoction for 24 weeks on the basis of the treatment in the control group. The changes of TCM syndrom score, liver function, blood routine, coagulation function index, Child-Pugh score, abdominal circumference, weight, and 24-hour urine output of the two groups of patients were observed. Results: The clinical effective rates of the patients in the treatment group and the control group were 81.8%(27/33) and 58.1%(18/31), the treatment group was significantly higher than the control group(P<0.05);the Child-Pugh scores of the patients in the treatment group were lower than the control group(P<0.05), the improvement of ascites, reduction of the total bilirubin, abdominal circumference and weight, increase of urine output were significantly better than the control group(P<0.05). Conclusion: Kangshi Daoshui Fuling Decoction has definite curative effect on patients with water dampness stagnation type decompensated alcoholic cirrhosis, and its mechanism may be related to reducing liver inflammation and improving hepatocyte metabolism.
