摘  要:目的:研究采用分子杂交技术检测动脉粥样硬化性心血管疾病(ASCVD)患者血标本中CYP2C192/17基因多态性分布及其价值。方法:331例ASCVD患者,采用分子杂交技术检测与氯吡格雷药物代谢相关基因CYP2C1917和CYP2C192多态性,一代测序方法验证分子杂交技术的准确性,并对试剂稳定性进行评价。结果:331例患者中CYP2C192/17基因存在超快代谢型、快代谢型、中间代谢型和慢代谢型4种药物代谢型,中间代谢型和慢代谢型共占51.36%,可能会影响氯吡格雷药效。20例样本的基因测序与分子杂交结果完全一致,分子杂交技术检测准确性为100%。试剂冻融稳定性及加速稳定性均符合要求。结论:相当部分ASCVD患者存在CYP2C192/17基因中间代谢型或慢代谢型,影响氯吡格雷的应用。分子杂交技术是一种快速、简便、准确性高的检测CYP2C192/17基因多态性方法,尤其适用于基层医院。Objective:To explore molecular hybridization technique to detect CYP2C192/17 gene polymorphism and value in ASCVD.Methods:Molecular hybridization technique was used to detect CYP2C1917 and CYP2C192 genes polymorphisms in 331 cases of ASCVD patients.The generation sequencing method is applied to verify the accuracy of molecular hybridization,and the stability of reagent is evaluated.Results:There are four drug metabolisms in CYP2C192/17 gene in 331 cases of ASCVD patients:super fast,fast,intermediary and slow metabolisms.Intermediary and slow metabolism accounted for 51.36%,and may affect the efficacy of clopidogrel.The results of gene sequencing and molecular hybridization of 20 cases were completely identical,and the detection accuracy of molecular hybridization is 100%.Reagent freeze-thaw stability and accelerated stability all meet the requirements.Conclusion:Quite a number of ASCVD patients have intermediary or slow metabolism in CYP2C192/17 gene,which affects the application of clopidogrel.Molecular hybridization technique is a rapid,simple and accurate method for detecting CYP2C192/17 gene polymorphism,which is especially suitable for primary hospitals.
