双联支气管舒张剂联合三子止咳胶囊治疗稳定期COPD的临床效果观察

摘　　要：目的探讨双联支气管舒张剂[长效抗胆碱能药物(LAMA)+长效β2受体激动剂(LABA)]联合三子止咳胶囊应用于稳定期慢性阻塞性肺疾病(COPD)治疗中的临床疗效及对患者肺功能的影响。方法选取大连市某医院2019年1月—2021年5月诊治的86例稳定期COPD患者为研究对象。采用随机数字表法将患者分为观察组与对照组,每组43例。对照组患者进行乌美溴铵/维兰特罗吸入粉雾剂治疗,观察组患者在对照组基础上加用三子止咳胶囊。比较2组患者治疗前后专科疾病自我评估测试量表(CAT)评分;比较2组患者第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC、还原型谷胱甘肽(GSH)、丙二醛(MDA)以及超氧化物歧化酶(SOD)水平。比较2组患者不良反应发生情况。结果治疗前,2组患者CAT评分及FEV_(1)、FVC、FEV_(1)/FVC、GSH、MDA、SOD水平比较,差异无统计学意义(P>0.05)。治疗后,2组患者CAT评分、MDA水平低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05);而2组患者FEV_(1)、FVC、FEV_(1)/FVC、GSH、SOD水平均高于治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05)。2组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论LAMA/LABA联合三子止咳胶囊应用于稳定期COPD治疗中的效果显著,可有效改善患者肺功能,减轻患者机体氧化应激反应,且安全性较高,值得临床推广。Objective To investigate the clinical efficacy of dual bronchodilators[long-acting muscarinic antagonists(LAMA)+long-actingβ2 agonist(LABA)]combined with Sanzi Zhike capsule in the treatment of stable chronic obstructive pulmonary disease(COPD)and its effects on the patients′pulmonary function and adverse reactions.Methods Eighty-six patients with stable COPD treated in a hospital in Dalian from January 2019 to May 2021 were selected for the study.The random number table method was used to divide the patients into observation and control groups,with 43 cases in each group.Patients in the control group were treated with umeclidinium-vilanterol powder for inhalation,and patients in the observation group were treated with Sanzi cough capsules on top of the control group.The scores of the COPD Assessment Tool(CAT)were compared before and after treatment;the forced expiratory volume in the first second(FEV_(1)),forced vital capacity(FVC),FEV_(1)/FVC,and the levels of reduced glutathione(GSH),malondialdehyde(MDA)and superoxide dismutase(SOD)were compared between the two groups.The incidence of adverse reactions was compared between the two groups.Results Before treatment,there was no statistically significant difference in CAT scores,FEV_(1),FVC,FEV_(1)/FVC and GSH,MDA and SOD levels between the 2 groups(P>0.05).After treatment,CAT scores and MDA levels of patients in the two groups were lower than those before treatment and lower in the observation group than in the control group,with statistically significant differences(P<0.05),while the FEV_(1),FVC,FEV_(1)/FVC and GSH and SOD levels of patients in the two groups were higher than those before treatment and higher in the observation group than in the control group,with statistically significant differences(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the 2 groups(P>0.05).Conclusion LAMA/LABA combined with Sanzi cough capsules is effective in the treatment of stable COPD.It can effectively improve the pulmo
