缺血性脑卒中伴吞咽障碍患者对出院计划服务需求的质性研究

摘　　要：目的探讨缺血性脑卒中吞咽障碍患者对出院计划服务的需求,为制订科学、规范的出院计划提供参考依据。方法采用质性研究中的现象学研究方法。选取2020年12月至2021年6月在广州医科大学附属第二医院神经内科住院的10例缺血性脑卒中吞咽障碍患者(男9例,女1例,年龄43~82岁,学历小学至大专,住院4~24 d),进行半结构式访谈,并用Colaizzi分析法对资料进行编码、分析、归类和提炼主题。结果卒中吞咽患者的出院计划服务需求归纳为4个主题,分别为疾病知识、吞咽康复锻炼、精神心理支持、社会支持需求。结论缺血性脑卒中吞咽障碍患者对出院计划服务需求较高,应充分发挥医护技、家属的协同作用,提供个性化多学科协作的出院计划服务,提高缺血性卒中吞咽障碍患者出院后的生活质量。Objective To explore the needs of patients with ischemic stroke and dysphagia for discharge planning services,and to provide references for formulating a scientific and standardized discharge plan.Methods The phenomenological research methods were used in the qualitative research.Ten ischemic stroke patients with dysphagia hospitalized in Neurology Department,The Second Affiliated Hospital of Guangzhou Medical University for 4-24 d from December 2020 to June 2021 were selected for semi-structured interview,including 9 males and 1 female.They were 43-82 years old.Their education level was from elementary school to college.The Colaizzi analysis method was used to code,analyze,classify,and refine the data.Results The needs for discharge planning services of the stroke patients with dysphagia were summarized into four themes:disease knowledge,swallowing rehabilitation exercise,mental and psychological support,and social support.Conclusions The needs for discharge planning services of stroke patients with dysphagia are high.The doctors,nurses,technologists,and family members should work together so as to provide individualized multi-disciplinary cooperative discharge planning services for the patients and improve their life quality after discharge.
