摘  要:目的:探讨雌、孕激素在CD34^(+)脐血来源的造血干细胞(hematopoietic stem cells,HSCs)的增殖中的作用。方法采用流式细胞术检测妊娠女性(n=26)及未孕女性(n=24)外周血中HSCs占单个核细胞的比率及造血祖细胞不同亚群占比。建立体外模型,从脐静脉血(n=36)中收集CD34^(+)HSCs进行体外扩增培养,给予50 ng/mL雌二醇或500 ng/mL孕酮进行刺激,以不加激素的作为对照组,采用细胞计数试剂盒及流式细胞术检测HSCs增殖情况。结果与未孕女性相比,妊娠女性外周血的HSCs比率明显增高(P<0.05),普通髓系祖细胞占多能祖细胞(MPPs)的比率明显降低(P<0.01),巨核细胞-红细胞祖细胞及粒细胞-巨细胞祖细胞占MPPs的比率增高(P<0.05)。与未给药组相比,雌、孕激素分别作用72 h后,HSCs的增殖指数均明显增高(均P<0.01),雌、孕激素分别作用7 d后,Lin-CD34^(+)HSCs和Lin-细胞流式细胞计数均增高(均P<0.05)。结论女性妊娠期间造血活动明显增强且明显向红系细胞和髓系细胞分化偏移,雌、孕激素可促进CD34^(+)脐血来源的HSCs增殖。Objective:To investigate the role of estrogen and progesterone in proliferation of CD34^(+)hematopoietic stem cells(HSCs)derived from cord blood.Methods Flow cytometry was used to measure the ratio of HSCs to mononuclear cells and the percentages of different subgroups of hematopoietic progenitor cells in the peripheral blood of pregnant women(n=26)and non-pregnant women(n=24).An in-vitro model was established,in which,CD34^(+)HSCs collected from umbilical venous blood(n=36)were subjected to in-vitro expansion and culture,stimulated with 50 ng/mL estradiol or 500 ng/mL progesterone.A hormone-free set was used as the control group.Cell counting kit and flow cytometry were used to examine the HSC proliferation.Results Compared with non-pregnant women,the pregnant women showed significantly higher proportion of HSCs in the peripheral blood(P<0.05),lower percentage of common myeloid progenitor in multipotent progenitors(MPPs)(P<0.01),higher percentages of megakaryocyte-erythrocyte and granulocyte-giant cell progenitors in MPPs(P<0.05).Compared with the non-treated group,the HSC proliferation index was significantly increased after treatment with estrogen and progesterone for 72 hours(bothP<0.01).After treatment with estrogen and progesterone for 7 days,the counts of Lin-CD34^(+)HSCs and Lin-cells were increased in flow cytometry(allP<0.05).Conclusion The During pregnancy,women present enhanced hematopoiesis with a significant shift to erythroid and myeloid cells.Estrogen and progesterone can promote the proliferation of CD34^(+)HSCs derived from cord blood.
