摘 要:目的:观察经食管实时三维超声心动图(RT3D-TEE)评估非瓣膜性房颤(NVAF)患者左心耳(LAA)功能的价值。方法:选取2020年1月至2021年3月在我院住院并行RT3D-TEE检查的患者200例。根据临床诊断结果分为NVAF组(123例)与非AF组(77例),所有患者均接受经胸超声心动图(TTE)检测和RT3D-TEE检测,分析对比两组患者的TTE、RT3D-TEE检测结果,并对比不同检查方法在NVAF患者LAA功能方面的评估价值。结果:TTE检测结果:两组IVSd、左室收缩末期容积(LVESV)、HR对比无统计学差异(P>0.05);NVAF组左心房前后径(LAD)、室间隔厚度(IVSd)、左心房容积(LAV)、左室舒张末期容积(LVEDV)明显大于非AF组而左室射血分数(LVEF)、二尖瓣血流E峰速度/A峰速度(E/A)明显小于非AF组,对比有统计学差异(P<0.05)。RT3D-TEE检测结果:NVAF组左心耳开口最大内径(LAA-Dmax)、开口最小内径(LAA-Dmin)、最大开口面积(LAA-Amax)、最小开口面积(LAA-Amin)、舒张末期最大容积(LAAv-3Dmax)、收缩末期最小容积(LAAv-3Dmin)均明显大于非AF组而内径变化率(LAA-D%)、面积变化率(LAA-A%)、LAA射血分数(LAAEF-3D)、排空速度(LAA-EV)、充盈速度(LAAFV)明显小于非AF组,对比有统计学差异(P<0.05)。结论:RT3D-TEE更能准确评估NVAF患者的左心耳功能。Objective:To observe the value of real-time three-dimensional transesophageal echocardiography(RT3D-TEE)in evaluating left atrial appendage(LAA)function in patients with non-valvular atrial fibrillation(NVAF).Methods:A total of 200 patients who were admitted to the hospital between January 2020 and March2021 were selected as the research subjects and 200 patients were concurrently examined with RT3D-TEE.According to the clinical diagnostic results,the patients enrolled were divided into NVAF group(123 cases)and non-AF group(77 cases).All patients were subjected to transthoracic echocardiography(TTE)and RT3D-TEE.The value of different examination methods in evaluating LAA function in patients with NVAF was compared.Results:TTE found that there was no statistically significant difference in interventricular septum dimension(IVSd),left ventricular end-systolic volume(LVESV)and heart rate(HR)between the two groups(P>0.05).The left atrial anterior and posterior diameters(LAD),left atrial volume(LAV)and left ventricular enddiastolic volume(LVEDV)in NVAF group were significantly greater than those in non-NVAF group,while left ventricular ejection fraction(LVEF)and early diastolic transmitral velocity to early mitral anulus diastolic velocity ratio(E/A)were significantly smaller than those in non-NVAF group,with statistically significant differences(P<0.05).RT3D-TEE found that LAA-maximum inner diameter of opening(LAA-Dmax),LAA-minimum inner diameter of opening(LAA-Dmin),LAA-maximum opening area(LAA-Amax),LAA-minimumopening area(L(AA-Amin),LAA-maximum end diastolic volume(LAAv-3Dmax)and LAA-minimum end-systolic volumeLAAv-3Dmin)in NVAF group were significantly greater than those in the non-NVAF group,while LAA-inner diameter change rate(LAA-D%),(LAA-area change rate(LAA-A%),LAA-ejection fractionLAAEF-3D),LAA-emptying velocity(LAA-EV)and LAA-filling velocity(LAA-FV)were significantlysmaller than those in non-NVAF group,with statistically significant differences(P<0.05).Conclusion:RT3D-TEE Kis emy owreo radccsurate to eval
