摘　　要：原发性中枢神经系统血管炎(PACNS)是一种罕见且临床表现复杂的中枢神经系统炎性疾病,其总体预后较差,极易复发。目前尚无涉及运用中医理论探讨PACNS病因病机及治疗,文章兹从中医相关理论出发,探讨中医脾肾与PACNS的密切联系,进一步阐明脾肾亏虚是该病缓解期最基本的病机特点——"脾肾亏虚日久,气血化生及运行受阻,正虚感邪,邪毒留滞,循络入脑,损伤脑脉,引动旧疾复燃",进而强调"补脾益肾"为重要治疗原则,临床上力求达到正气留存,驱邪外出,减少该病的复发率,以期为PACNS的中医治疗开拓新思路。Primary central nervous system vasculitis is a rare and complex inflammatory disease of the central nervous system.Its overall prognosis is poor and it is very easy to relapse.At present,the use of TCM theories to explore the etiology,pathogenesis and treatment of PACNS has not been involved.Starting from the related theories of TCM,we will discuss the close connection between the spleen and kidney of TCM and PACNS,and further clarify that the deficiency of the spleen and kidney is the most basic pathogenesis feature of the disease in remission—"The spleen and kidney are deficient for a long time,the metaplasia and movement of Qi and blood are hindered,the body is weak and the evil is sensed,the evil toxin is stagnated,the collaterals enter the brain,the brain is damaged,and the old diseases are revived.Furthermore,it emphasizes the important treatment principle of"tonifying the spleen and kidney".Clinically,it strives to achieve the retention of righteous Qi,expel evil spirits and reduce the recurrence rate of the disease,in order to open up new ideas for the treatment of PACNS in Chinese medicine.
