负重位磁共振成像与下肢全长X线片评估前交叉韧带损伤患者胫骨前移程度的对比研究

摘　　要：目的 探讨负重位磁共振成像(MRI)与负重位下肢全长X线片(EOS)在评估前交叉韧带(anterior cruciate ligament,ACL)损伤患者的胫骨前移程度方面的差异和相关性。方法 对44例ACL损伤患者在24h内,分别进行负重位MRI和负重位EOS检查,其中术前23例,术后21例,测量两种检查的胫骨前移距离并比较两者的差异及相关性。结果 44例患者负重位的MRI胫骨前移平均距离为(8.51±4.31)mm,EOS测量胫骨前移距离为(7.16±4.88)mm,差异有统计学意义(P<0.05),两者相关系数R;=0.6885,P<0.05;23例术前患者MRI胫骨前移距离为(8.00±4.37)mm,EOS胫骨前移距离为(6.36±4.66)mm,差异有统计学意义(P<0.05),两者相关系数R^(2)=0.6254,P<0.05。21例术后患者MRI胫骨前移距离为(9.06±4.29)mm,EOS胫骨前移距离为(8.06±5.07)mm,差异无统计学意义(P=0.085),两者相关系数R^(2)=0.7514,P<0.05。结论 对于ACL损伤患者,负重位MRI与EOS在评估胫骨前移程度方面相关性好,特别是在术后患者中,两者的测量值差异无统计学意义。Objective To investigate the difference and relationship between weight-bearing MRI and EOS in the measurement of tibial anterior translation (TAT) in patients with anterior cruciate ligament (ACL) injury.Methods TAT of 44 patients with ACL injury were examined by weight-bearing MRI and EOS within 24 hours after injury,including 23 patients before surgery and 21 patients after surgery.Differences were compared.Results Statistically significant differences in TAT of 44 patients in total[(8.51±4.31) mm,(7.16±4.88) mm;R^(2)=0.6254,P < 0.05]and 23 patients before surgery[(8.00±4.37) mm,(6.36±4.66) mm;R^(2)=0.6254,P < 0.05]were noted between the results measured by MRI and EOS,but not for 21 patients after surgery[(9.06±4.29) mm,(8.06±5.07) mm;R^(2)=0.7514,P > 0.05].Conclusions Both weight-bearing MRI and EOS could be applied in the measurement of TAT in patients with ACL injury.However,no statistically significant differences are observed for patients after surgery.
