梅花针扣刺联合龙胆泻肝汤治疗带状疱疹后遗神经痛临床研究

摘　　要：目的:观察梅花针扣刺联合龙胆泻肝汤治疗带状疱疹后遗神经痛的临床疗效及对患者血清β-内啡肽(β-EP)、P物质(SP)及前列腺素E2(PGE-2)表达的影响。方法:选取带状疱疹后遗神经痛患者292例作为研究对象,按随机数字表法分为对照组和观察组各146例。对照组给予常规西药和龙胆泻肝汤口服,观察组在对照组基础上加用梅花针扣刺治疗。比较2组临床疗效、疼痛视觉模拟量表(VAS)评分、生命质量(QOL)评分、血清SP、PGE-2、β-EP水平,观察2组止疱、结痂情况。结果:观察组总有效率为97.26%,高于对照组79.45%,差异有统计学意义(χ_(2)=20.805,P<0.05)。治疗后,2组VAS评分较治疗前降低,且观察组VAS评分低于对照组(P<0.05);2组QOL评分较治疗前升高,且观察组QOL评分高于对照组(P<0.05)。治疗后,2组血清SP、PGE-2水平较治疗前降低,且观察组血清SP、PGE-2水平低于对照组(P<0.05);2组血清β-EP水平较治疗前升高,且观察组血清β-EP水平高于对照组(P<0.05)。观察组止疱时间、结痂时间短于对照组(P<0.05)。结论:梅花针扣刺联合龙胆泻肝汤治疗带状疱疹后遗神经痛可有效降低血清SP、PGE-2水平,提高β-EP水平,减轻患者疼痛,改善生活质量,促进患者康复,疗效显著。Objective:To observe the clinical effect of plum-blossom needle tapping combined with Longdan Xiegan tang for postherpetic neuralgia,and its effect onβ-endorphin(β-EP),substance P(SP)and prostaglandin E2(PGE2)in serum.Methods:A total of 292 cases of patients with postherpetic neuralgia were selected as the study subjects,and divided into the control group and the observation group according to the random number table method,with 146 cases in each group.The control group was given oral administration of routine western medicine and Longdan Xiegan tang,and the observation group was additionally treated with plum-blossom needle tapping based on the treatment of the control group.The clinical effects,Visual Analogue Scale(VAS)scores,Quality of Life(QOL)scores,and levels of SP,PGE-2 andβ-EP in serum in the two groups were compared.The condition of stopping vesiculation and forming scab in the two groups were observed.Results:The total effective rate was 97.26%in the observation group,higher than that of 79.45%in the control group,the difference being significant(χ_(2)=20.805,P<0.05).After treatment,the VAS scores in the two groups were decreased when compared with those before treatment,and the VAS score in the observation group was lower than that in the control group(P<0.05);the QOL scores in the two groups were increased when compared with those before treatment,and the QOL score in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of SP and PGE-2 in serum in the two groups were decreased when compared with those before treatment,and the levels of SP and PGE-2 in serum in the observation group were lower than those in the control group(P<0.05);the levels ofβ-EP in serum in the two groups were increased when compared with those before treatment,and the level ofβ-EP in serum in the observation group was higher than that in the control group(P<0.05).The time of stopping vesiculation and forming scab in the observation group was shorter than that in the control gro
