脊柱内镜治疗伴有稳定型椎体滑脱的老年腰椎管狭窄症19例

摘　　要：目的:评价应用大直径脊柱内镜单侧椎板间隙入路双侧椎管减压方式治疗伴有稳定型椎体滑脱的老年腰椎管狭窄症的临床疗效。方法:收集自2019年3月至2020年6月应用大直径脊柱内镜单侧椎板间隙入路双侧椎管减压治疗伴有稳定型椎体滑脱的老年腰椎管狭窄症患者19例,记录手术时间、术前术后疼痛VAS评分、JOA评分及术后并发症,末次随访时采用改良MacNab标准评价疗效。结果:手术时间为80~150 min,平均为(105.0±27.3)min,未见严重术后并发症,术后随访各时间点VAS评分与JOA评分均优于术前,差异有统计学意义(P<0.05)。末次随访MacNab评分评定优良率为89.47%。结论:大直径脊柱内镜单侧椎板间隙入路双侧椎管减压方式可用于治疗伴有稳定型椎体滑脱的老年腰椎管狭窄症。Objective:To evaluate clinical efficacy of unilateral laminectomy for bilateral decompression under large-diameter spinal endoscope on the treatment of senile lumbar spinal stenosis with stable spondylolisthesis. Methods:19 senile lumbar spinal stenosis with stable spondylolisthesis from March 2019 to June 2020 were treated with unilateral laminectomy for bilateral decompression under large-diameter spinal endoscope were selected. The operation time, preoperative and postoperative pain VAS score, JOA score and postoperative complications were recorded. The curative efficacy was evaluated by the modified MacNab standard at the last follow-up. Results:The operation time was 80 to 150 min, with an average of(105.0±27.3) min, and no serious postoperative complications were found. The VAS score and JOA score at each time point of postoperative follow-up were better than those before operation(P<0.05). The excellent and good rate of MacNab score in the last follow-up was 89.47%. Conclusion:Unilateral laminectomy for bilateral decompression under large-diameter spinal endoscope can be used to treat senile lumbar spinal stenosis with stable spondylolisthesis.
