摘　　要：目的探讨延长COOK宫颈扩张球囊放置时间对足月妊娠分娩结局的影响。方法回顾性分析南方医科大学第七附属医院产科2019年5月1日至2020年5月1日妊娠37~42周、无阴道试产禁忌证、并且宫颈Bishop评分≤6分的正常妊娠者143例,均采用一次性COOK宫颈扩张球囊扩张器促宫颈成熟。按照水囊拔除时间分为对照组83例(12 h后拔除水囊,进行宫颈评分,然后静脉滴注催产素)与研究组60例(18 h后拔除水囊,进行宫颈评分,然后静脉滴注催产素),分析两组宫颈成熟及引产效果、分娩方式、妊娠合并症等。结果对照组83例阴道分娩成功47例(56.63%),宫颈评分提高有效62例(74.69%,62/83),感染2例(2.41%,2/83),产后出血5例(6.02%,5/83);研究组60例阴道分娩成功46例(76.66%,46/60),宫颈评分提高有效55例(91.67%,55/60),感染2例(3.33%,2/60),产后出血3例(5.00%,3/60)。结论适当延长放置COOK宫颈扩张球囊时间可明显提高宫颈Bishop评分,提高经阴道分娩率,降低剖宫产率,并且不增加引产并发症。Objective To investigate the effect of prolonged placement of COOK cervical dilatation balloon on delivery outcome of full-term pregnancy. Methods A retrospective analysis was performed on 143 cases of normal pregnancy in the Seventh Affiliated Hospital of Southern Medical University from May 1,2019 to May 1,2020 who were 37-42 weeks of gestation without conjunctive certificate of vaginal trial labor and had a cervical Bishop score ≤ 6. All cases were treated with disposable COOK cervical dilatation balloon dilator to promote cervical maturity. According to the time of water sac extraction,83 cases were recruited into the control group( water sac was removed 12 h later,cervical score was performed,and then oxytocin was dropped),and 60 cases were recruited into in the research group( water sac was removed 18 h later,cervical score was performed,and then oxytocin was dropped). The cervical maturity and induced labor effect,delivery mode,pregnancy complications of the two groups were analyzed. Results In the control group,47 cases( 56. 63%) were successful in vaginal delivery,62 cases( 74. 69%) were effective in improving cervical score,2 cases( 2. 41%) were infected,5 cases( 6. 02%) were postpartum hemorrhage. In the study group,46 cases( 76. 66%)were successful in vaginal delivery,55 cases( 91. 66%) were effective in improving cervical score,2 cases( 3. 33%) were infected, and 3 cases( 5. 00%) were postpartum hemorrhage.Conclusions Properly prolonged placement of COOK cervical dilatation balloon can significantly improve the cervical Bishop score,increase the rate of vaginal delivery,reduce the rate of cesarean section,without increasing the complications of labor induction.
