补肾接骨方联合带锁髓内钉内固定术治疗四肢骨折后骨不连临床研究

摘　　要：目的:观察补肾接骨方联合带锁髓内钉内固定术治疗四肢骨折后骨不连的临床疗效。方法:选取82例四肢骨折后骨不连患者,根据随机数字表法分为对照组和观察组,每组41例。对照组使用带锁髓内钉内固定术治疗,术后给予抗生素等常规治疗,观察组在对照组基础上给予补肾接骨方治疗。比较2组术前及术后3个月血清碱性磷酸酶(ALP)水平、术前及术后9个月骨痂评分,比较2组骨折愈合时间、骨不连愈合率及临床疗效。结果:术后3个月,2组血清ALP水平均较术前升高(P<0.05),观察组血清ALP水平高于对照组(P<0.05)。术后9个月,2组骨痂评分均较术前升高(P<0.05),观察组骨痂评分高于对照组(P<0.05)。观察组骨折愈合时间为(6.51±1.05)个月,短于对照组的(7.82±1.15)个月(P<0.05)。术后9个月,观察组骨不连愈合率95.12%,高于对照组78.05%(P<0.05);观察组临床疗效优于对照组(P<0.05)。结论:补肾接骨方联合带锁髓内钉内固定术治疗四肢骨折后骨不连,可缩短患者的骨折愈合时间,提高骨不连愈合率。Objective:To observe the clinical effect of Bushen Jiegu prescription combined with internal fixation with interlocking intramedullary nail for nonunion after limb fracture.Methods:A total of 82 cases of patients with nonunion after limb fracture were selected and divided into the control group and the observation group according to the random number table method,with 41 cases in each group.After internal fixation with interlocking intramedullary nail,the control group was given antibiotics for routine treatment,and the observation group was additionally treated with Bushen Jiegu prescription based on the treatment of the control group.Before operation and three months after operation,the levels of alkaline phosphatase(ALP) in serum in the two groups were compared.Before operation and nine months after operation,the scores of fracture callus in the two groups were compared.The fracture healing time,the healing rate of nonunion,and the clinical effects in the two groups were compared.Results:Three months after operation,the ALP levels in serum in the two groups were increased when compared with those before operation(P<0.05),and the ALP level in serum in the observation group was higher than that in the control group(P<0.05).Nine months after operation,the scores of fracture callus in the two groups were increased when compared with those before operation(P<0.05),and the score of fracture callus in the observation group was higher than that in the control group(P<0.05).The fracture healing time was(6.51±1.05) months in the observation group,shorter than that of(7.82±1.15) months in the control group(P<0.05).Nine months after operation,the healing rate of nonunion was 95.12% in the observation group,higher than that of 78.05% in the control group(P<0.05).The clinical effect in the observation group was better than that in the control group(P<0.05).Conclusion:Bushen Jiegu prescription combined with internal fixation with interlocking intramedullary nail for nonunion after limb fracture can shorten the fracture hea
