摘　　要：介绍陈雷主任中医师运用外治法治疗带状疱疹后遗神经痛临床经验。陈雷主任结合发病人群特性、病程衍变特征和疾病症状特点等方面,提出带状疱疹后遗神经痛以气阴两虚兼气滞血瘀最为多见,同时因带状疱疹前期和中期遗留邪气的不同,在后遗症期兼见湿热蕴结、肝火偏亢等。在治疗上主用外治方法,立足扶正复机体之耗,祛瘀解疼痛之因等原则,采用精准取穴、辨证用针、围针平刺、随取阿是、针罐结合、调神宁心等方法,不忘扶正、祛瘀,兼顾兼证。This paper introduces the clinical experience of chief Chinese medicine physician CHEN Lei applying external therapy to the treatment of postherpetic neuralgia of herpes zoster.In combination with the characteristics of the affected population,the development of the course of disease and symptoms,CHEN puts forward that most patients with postherpetic neuralgia of herpes zoster were in conditions of qi-yin deficiency and qi stagnation and blood stasis;at the same time,due to the different evil qi left in the early and middle stages of herpes zoster,there is the accumulation of dampnessheat and intense liver fire during the sequelae period.External treatment is the main choice,with the principle of reinforcing the healthy qi to supplement the body and dispelling stasis and relieving pain;accurate point selection,needling based on syndrome differentiation,transverse insertion with encircled needling,flexible use of ouch point,the combination of cupping and needling,the method of calming the mind and heart,and other methods can be applied to the treatment of the disease,and the attention should be paid to reinforcing the healthy qi,dispelling stasis and treating secondary syndromes.
