摘 要:目的探讨MRI对Lemmel综合征的诊断价值。方法选取66例临床确诊为Lemmel综合征患者的MRI资料进行回顾性分析,观察憩室的位置,评估其与肝胆胰疾病关系。结果66例患者中发现十二指肠乳头旁憩室(PAD)66个,其中乳头上型61个,憩室内型5个;伴胆管扩张45例,胆管炎28例,胆总管结石20例,胆囊结石9例,胆囊炎15例,胰腺炎8例,胰管扩张2例,肝内胆管细胞癌1例。结论MRI可以明确诊断PAD,并清晰显示其相对十二指肠乳头的位置,为评估其与肝胆胰疾病的关系并诊断Lemmel综合征提供参考。Objective To evaluate the value of MRI in the diagnosis of Lemmel syndrome.Methods MRI data of 66 patients with Lemmel syndrome were analyzed retrospectively.The location of diverticulum was observed and the relationship between diverticulum and hepatobiliary and pancreatic diseases was evaluated.Results A total of 66 parapapillary duodenal diverticula were found in 66 patients,including 61 suprapapillary diverticulum and 5 internal diverticulum,45 with bile duct dilatation,28 with cholangitis,20 with choledocholithiasis,9 with cholecystolithiasis,15 with cholecystitis,8 with pancreatitis,2 with pancreatic duct dilatation,and 1 with intrahepatic cholangiocarcinoma.Conclusion MRI can clearly diagnose PAD and show its position relative to duodenal papilla,which provides an important reference for evaluating its relationship with hepatobiliary and pancreatic diseases and diagnosing Lemmel syndrome.
