摘 要:目的通过兔肺激光消融实验,探讨CT图像中消融坏死区的范围。方法在CT引导下,采用激光仪对兔一侧肺组织进行激光消融(laser ablation,LA)。术中及术后观察消融后即刻并发症并记录。通过CT表现以及解剖后肺组织大体标本观察消融灶形态及范围,光镜下观察消融灶的病理性变化。结果激光光纤向前发射形成消融灶,呈类椭圆形。CT测量磨玻璃影范围平均为:800J(12.5 mm×9.1 mm);1200J(14.3 mm×12.5 mm),实际坏死区范围为:800J(8.0 mm×6.6 mm);1200J(10.1 mm×7.6 mm)。消融灶范围高能量组大于低能量组,其差异具有统计学意义(P<0.05)。CT显示消融灶范围较有效消融灶测量范围大,两者比较差异具有统计学意义(P<0.05)。结论CT图像中消融灶范围与病理相比具有一定的差异性,通过本实验能够更为准确地了解肺部激光消融灶坏死区的范围。Objective The laser ablation experiment of rabbit lung was conducted to investigate the range of ablation necrosis in CT images.Methods Under the guidance of CT,laser ablation(LA)was performed on one side of the rabbit lung.Intraoperative and postoperative complications immediately after ablation were observed and recorded.The morphology and range of the ablation zone were observed by CT images and gross specimens of lung tissue after anatomy,and the pathological changes of the ablation zone were observed under light microscopy.Results Coagulation caused by laser ablation.The average range of ground-glass opacity measured by CT was 800J(12.5 mm×9.1 mm).1200J(14.3 mm×12.5 mm),the actual necrotic area was 800J(8.0 mm×6.6 mm);1200J(10.1 mm×7.6 mm).The range of ablation zone in the high-energy group was greater than that in the low-energy group(P<0.05).CT measurement of the ablation zone was bigger than the effective ablation(P<0.05).Conclusion The range of ablation zone in CT images is different from that in pathology.This experiment can accurately understand the range of necrosis area in laser ablation.
