摘 要:目的:了解广东省4城市高危职业人群布鲁氏菌感染状况,探讨其布鲁氏菌感染的相关因素。方法:2013年1—6月,采用整群抽样的方法从广东省广州、佛山、惠州和揭阳4个城市招募了1 050名畜牧业从业人员为研究对象,对其进行问卷调查和血清学检查。结果:调查人群血清阳性率为1.5%(16/1 050),其中牲畜交易市场从业人员阳性率最高(3.1%,6/195),其次是屠夫(2.2%,6/269)、农民或奶工(2.1%,4/191)。多因素分析表明接触绵羊、山羊或其产品、过去12个月内有持续关节痛者和年龄小是职业人群中布鲁氏菌感染血清阳性的相关因素(校正OR=10.5、26.8、17.5)。与接触牛或猪的工人相比,接触绵羊、山羊或其产品者的布鲁氏菌血清阳性率更高(分别为5.5%和0.6%,校正OR值=10.5;95%CI:3.3~33.7)。结论:广东省4城市畜牧业从业人员布鲁氏菌感染血清阳性率相对较低,接触绵羊、山羊或其产品、过去12个月内有持续关节痛者和年龄小是主要的相关因素。Objective:To investigate the seroprevalence of Brucella infection among high-risk occupational populations in 4 cities of Guangdong Province,and to explore the related factors of Brucella infection.Methods:From January to June 2013,1 050 animal husbandry practitioners were recruited from 4 cities of Guangzhou,Foshan,Huizhou and Jieyang in Guangdong Province by cluster sampling as research subjects,and they were subjected to questionnaire survey and serological examination.Results:The seropositivity rate of the surveyed population was 1.5%(16/1 050),among which the seropositivity rate of the livestock market practitioners was the highest(3.1%,6/195),followed by butchers(2.2%,6/269),farmers or dairy workers(2.1%,4/191).Multivariate analysis showed that exposure to sheep,goats or their products,persistent arthralgia in the past 12 months,and younger age were associated with seropositivity for Brucella infection in the occupational population(adjusted OR=10.5,26.8,17.5).Compared with workers exposed to cattle or pigs,those exposed to sheep,goats or their products were more seropositive for Brucella(5.5% vs 0.6%,respectively,adjusted OR=10.5;95% CI:3.3-33.7).Conclusion:The seroprevalence of Brucella infection among animal husbandry workers in 4 cities in Guangdong Province was relatively low,and exposure to sheep,goats or their products,persistent arthralgia in the past 12months,and younger age were the main associated factors.
