摘 要:目的探讨冠心病患者血清胱抑素C(Cys-C)、同型半胱氨酸(Hcy)、γ谷氨酰转肽酶(γGT)水平与冠状动脉狭窄程度的关系。方法选取2017年6月至2019年12月在浙江衢化医院就诊的冠心病患者120例为冠心病组,另择同期在本院进行体检的健康体检者100例作为对照组。根据冠心病患者Gensini评分分为低危组、中危组及高危组,检测各组患者血清Cys-C、Hcy、γGT水平,采用Spearman法分析Cys-C、Hcy、γGT与冠心病患者冠状动脉狭窄程度的关系。结果与低危组相比,高危组和中危组血清TC、TG、HDL-C、Cys-C、Cys-C、γGT水平较高,且高危组高于中危组,而HDL-C水平较低,且高危组低于中危组(均P<0.05)。与对照组相比,冠心病组血清Cys-C、Hcy、γGT水平均升高(均P<0.01)。Cys-C、Hcy、γGT水平与冠状动脉狭窄程度均呈正相关(r=0.495、0655、0.728,均P<0.05);logistic回归分析显示,Cys-C、Hcy、γGT均为冠状动脉狭窄的危险因素(均P<0.05);Cys-C、Hcy、γGT均可预测冠状动脉狭窄程度,AUC分别为0.753、0.886、0.932,灵敏度分别为81.00%、72.00%、92.00%,特异度分别为64.17%、86.67%、91.84%。结论Cys-C、Hcy、γGT均可作为判断冠心病狭窄程度的独立标志物。Objective To investigate the correlation of cystatin C(Cys-C),homocysteine(Hcy)and γ-glutamyl transpepntidase(γGT)with the degree of coronary artery stenosis in patients with coronary heart disease(CHD).Methods One hundred and twenty patients with CHD treated in Zhejiang Quhua Hospital from June 2017 to December 2019(CHD group)and 100 healthy subjects who underwent health check-up in the same period(control group)were enrolled in the study.The patients were classified as low-risk group,medium risk group and high-risk group according to the Gensini score of coronary artery patients.The serum Cys-C,Hcy and γGT level were measured and their correlation with the severity of coronary heart disease was analyzed.Results Compared with the control group,serum Cys-C Hcy and γGT levels were increased(all P<0.01).Compared with low-risk group,serum TC,TG,HDL-C,Cys-C,Hcy andγGT levels in high-risk group was higher;and HDL-C level was lower(all P<0.05).The Cys-C,Hcy and γGT levels positively correlated with the degree of coronary artery stenosis(r=0.495,0655,0.728,all P<0.05).Logistic regression analysis showed that Cys-C,Hcy andγGT were the risk factors of coronary artery disease(all P<0.05).The areas under the ROC curve(AUC)of Cys-C,Hcy andγGT in predicting coronary artery disease were 0.753,0.886 and 0.932 respectively.The sensitivity is 81.00%,72.00% and 92.00% respectively,and the specificity is 64.17%,86.67% and 91.84% respectively.Conclusion Serum Cys-C,Hcy andγGT levels are increased in patients with coronary heart disease,which are correlated with the degree of coronary stenosis.The results suggest that serum Cys-C,Hcy andγGT levels may be used as independent markers to predict the severity of coronary heart disease.
