摘　　要：目的监测急性呼吸窘迫综合征(ARDS)患者呼出气冷凝液(EBC)和血清中细胞间黏附分子1(ICAM-1)的水平,探讨其临床意义。方法选取51例ARDS患者为ARDS组,根据柏林诊断标准分为3个亚组:轻度组(22例)、中度组(17例)、重度组(12例),选取同期51例健康体检人员为健康对照组,检测各组患者EBC和血清中ICAM-1的含量。根据ARDS患者入院28天时的生存情况,将ARDS患者分为存活组与死亡组,比较两组血清和EBC中ICAM-1含量。结果与健康对照组比较,ARDS组血清和EBC中ICAM-1显著增高(P<0.05)。入院第1天和第5天,不同严重程度ARDS患者血清及EBC中ICAM-1含量随着ARDS严重程度的加重而上升(P<0.05);中、重度组患者入院第1天血清中ICAM-1含量高于同组第5天(P<0.05);轻、中、重度组患者入院第1天EBC中ICAM-1含量均显著高于同组第5天(P<0.05)。死亡组入院第1天EBC中ICAM-1含量高于存活组(P<0.05),入院第5天时,血清和EBC中ICAM-1含量均显著高于存活组(P<0.05)。结论随着ARDS患者的严重程度不断升高,EBC和血清中ICAM-1水平不断升高。Aim To investigate the clinical significance of exhaled air condensate(EBC)and intercellular adhesion molecule-1(ICAM-1)in patients with acute respiratory distress syndrome(ARDS).Methods 51 ARDS patients were selected as ARDS group,which was divided into three subgroups according to Berlin diagnostic criteria:mild group(22 cases),moderate group(17 cases)and severe group(12 cases).51 healthy people in the same period were selected as healthy control group,and the levels of EBC and ICAM-1 in serum of each group were detected.According to the survival of ARDS patients at 28 days after admission,patients with ARDS were divided into the survival group and the death group,and the levels of ICAM-1 in serum and EBC of the two groups were compared.Results Compared with the healthy control group,the serum and EBC levels of ICAM-1 in ARDS group were significantly higher(P<0.05).On the first and fifth days of admission,the levels of ICAM-1 in serum and EBC of ARDS patients with different severity increased with the severity of ARDS(P<0.05);The level of ICAM-1 in serum of patients in the moderate and severe groups on the first day of admission was higher than that on the fifth day(P<0.05);The content of ICAM-1 in EBC in mild,moderate and severe groups was significantly higher than that in the same group on day 5(P<0.05).The level of ICAM-1 in EBC in the death group was higher than that in the survival group(P<0.05),the levels of ICAM-1 in serum and EBC were significantly higher than those in survival group(P<0.05).Conclusion With the increasing severity of ARDS patients,EBC and serum ICAM-1 levels increased.
