经皮穿刺低温等离子消融术与臭氧注射术治疗腰椎间盘突出症患者的效果比较

摘　　要：目的:比较经皮穿刺低温等离子消融术(TCRF)与臭氧(O_(3))注射术治疗腰椎间盘突出症(LDH)患者的效果。方法:选取118例LDH患者为研究对象,按照随机数字表法分为研究组与对照组各59例。对照组给予O_(3)注射术治疗,研究组给予经皮穿刺TCRF治疗。比较两组治疗优良率、手术时间、住院时间、治疗费用、视觉模拟评分法(VAS)评分和腰椎日本骨科协会评估治疗(JOA)评分。结果:研究组治疗优良率为84.75%,明显高于对照组的69.49%,差异有统计学意义(P<0.05);研究组手术时间和住院时间均长于对照组,治疗费用高于对照组,差异有统计学意义(P<0.05);治疗后1、3个月,研究组VAS评分均明显低于对照组,腰椎JOA评分明显高于对照组,差异有统计学意义(P<0.05)。结论:经皮穿刺TCRF治疗LDH患者可提高治疗优良率和腰椎JOA评分,以及降低VAS评分,效果优于O3注射术,但会延长手术时间、住院时间和提高治疗费用。Objective:To compare effects of percutaneous low-temperature plasma ablation(TCRF)and ozone(O_(3))injection in treatment of patients with lumbar disc herniation(LDH).Methods:118 patients with LDH were selected as the research objects and were divided into study group and control group according to the random number table method,59 cases in each.The control group was given O_(3) injection,while the study group was given percutaneous TCRF.The excellent and good rate of treatment,the operation time,the hospitalization time,the treatment costs,the visual analogue scale(VAS)score and the lumbar spine Japanese Orthopaedic Associationscores(JOA)score were compared between the two groups.Results:The excellent and good rate of treatment in the study group was 84.75%,which was significantly higher than the control group of 69.49%,and the difference was statistically significant(P<0.05).The operation time and the hospitalization time in the study group were longer than those in the control group;the treatment costs was higher than that in the control group;and the differences were statistically significant(P<0.05).Further,1 month and 3 months after the treatment,the VAS score of the study group was significantly lower than that of the control group;the JOA score of the lumbar spine was significantly higher than that of the control group;and the differences were statistically significant(P<0.05).Conclusions:Percutaneous TCRF in the treatment of the LDH patients can improve the excellent and good rate and the lumbar JOA score,and reduce the VAS score.However,it can prolong the operative time and the hospitalization time and increase the treatment costs.
