摘  要:目的探讨上海汉族人群中骨钙素基因单核苷酸多态性(SNP)的分布特征及其与2型糖尿病(T2DM)的关系。方法选取2019年12月到2021年1月就诊于上海市浦东新区浦南医院内分泌科的上海汉族T2DM患者和健康对照者。收集研究对象性别、年龄、身高、体重,并计算体重指数(BMI),收集其空腹C肽、空腹血糖(FPG)和空腹胰岛素(FINS)等指标,并计算稳态模型评估胰岛素抵抗指数(HOMA-IR)及稳态模型评估β细胞功能指数(HOMA-β)。提取研究对象DNA并对骨钙素基因9个位点(rs1543294、rs2277872等)进行基因分型,计算各基因型的分布频率。采用t检验、Mann‐WhitneyU检验或χ2检验对健康对照者与T2DM患者的基线资料进行比较,采用logistic回归分析法对不同模型(加性模型、共显性模型、显性模型、隐性模型)下两组研究对象骨钙素基因型的分布频率进行比较,采用线性回归分析法对骨钙素基因9个位点基因型与HOMA-β、HOMA-IR、空腹C肽和FINS的关联进行分析。对性别、年龄、身高、体重和BMI等因素进行校正。结果共纳入221例T2DM患者及113名健康对照者。两组研究对象不同骨钙素基因型在4个模型下的分布频率差异均无统计学意义(P>0.05);校正性别、年龄、身高、体重和BMI后,差异亦无统计学意义(P>0.05)。线性回归分析结果显示,T2DM患者骨钙素基因位点rs1543294 TT基因型与HOMA-β[OR值(95%CI)为359.10(173.20~545.00),P<0.001]、空腹C肽[OR值(95%CI)为7.68(4.06~11.29),P<0.001]及FINS[OR值(95%CI)为46.45(23.82~69.07),P<0.001]均相关;位点rs2277872 GG基因型与HOMA-β[OR值(95%CI)为354.40(159.30~549.50),P<0.001]、空腹C肽[OR值(95%CI)为8.12(4.38~11.93),P<0.001]及FINS[OR值(95%CI)为49.98(26.38~73.57),P<0.001]均相关。结论骨钙素基因rs1543294位点TT基因型、rs2277872位点GG基因型与上海汉族T2DM患者的HOMA-β、空腹C肽、FINS均相关,可能是骨钙素影响糖代谢的潜在基因型。Objective To explore the distribution characteristics of osteocalcin gene single nucleotide polymorphism in Shanghai Han population and its correlation with type 2 diabetes mellitus(T2DM).Methods Shanghai Han T2DM patients and healthy controls who were treated in the Department of Endocrinology,Punan Hospital,Pudong New Area,Shanghai from December 2019 to January 2021 were selected.The subjects′gender,age,height and weight were collected,and body mass index(BMI)was calculated.Fasting C-peptide,fasting plasma glucose(FPG),fasting insulin(FINS)and other indexes were collected.The homeostasis model assessment of insulin resistance(HOMA-IR)and homeostasis model assessment ofβ-cell function(HOMA-β)were calculated.The DNA of the subjects was extracted and nine loci of osteocalcin gene(rs1543294,rs2277872,etc.)were genotyped,and the distribution frequency of each genotype was calculated.The t test,Mann‐Whitney U test orχ2 test were used to compare the baseline data of healthy controls and T2DM patients.Logistic regression analysis was used to compare the distribution frequency of osteocalcin genotypes between the two groups under different models(additive,codominant,dominant and recessive).The correlations between nine genotypes of osteocalcin gene and(HOMA-β),(HOMA-IR),fasting C-peptide and FINS were analyzed by linear regression analysis.Factors such as gender,age,height,weight and BMI were adjusted.Results A total of 221 T2DM patients and 113 healthy controls were enrolled.There was no significant difference in the distribution frequency of different osteocalcin genotypes between the two groups under the four models(P>0.05).After adjusting for gender,age,height,weight and BMI,there was no significant difference(P>0.05).Linear regression analysis showed that the osteocalcin gene locus rs1543294 TT genotype in T2DM patients was associated with(HOMA-β)[OR(95%CI)was 359.10(173.20-545.00),P<0.001],fasting C-peptide[OR(95%CI)was 7.68(4.06-11.29),P<0.001]and FINS[OR(95%CI)was 46.45(23.82-69.07),P<0.001];locus rs227
