腓肠神经营养血管皮瓣与小腿内外侧皮瓣修复足踝部创面的效果对比

摘　　要：目的探讨足踝部创面使用腓肠神经营养血管皮瓣与小腿内外侧皮瓣修复的效果。方法60例足踝部创伤患者为研究对象,根据修复治疗方案的不同分为对照组及观察组,每组30例。对照组患者应用小腿内外侧皮瓣修复,观察组患者应用腓肠神经营养血管皮瓣修复。比较两组足踝部功能评分、手术时间、皮瓣修复情况(皮瓣长度、皮瓣宽度)、并发症发生情况、术后愈合情况。结果观察组足踝部功能评分(92.56±3.80)分高于对照组的(78.22±8.18)分,手术时间(1.17±0.08)h短于对照组的(2.22±0.12)h,差异具有统计学意义(P<0.05)。观察组的皮瓣长度(19.26±3.21)cm、皮瓣宽度(10.47±2.88)cm均长于对照组的(12.52±3.28)、(7.82±3.12)cm,差异具有统计学意义(P<0.05)。观察组并发症发生率3.33%低于对照组的20.00%,差异具有统计学意义(P<0.05)。观察组术后愈合率90.00%高于对照组的60.00%,差异具有统计学意义(P<0.05)。结论腓肠神经营养血管皮瓣修复对足踝创面的治疗效果更好,对比小腿内外侧皮瓣修复更具有治疗价值,值得推广。Objective To discuss the effect of sural nerve nutrient vessel flap and medial and lateral leg flap in the repair of ankle and foot wounds.Methods A total of 60 patients with foot and ankle wound were selected as the research subjects,and they were divided into control group and observation group according to different repair treatment regimens,with 30 cases in each group.Patients in the control group were repaired with medial and lateral leg flaps,and patients in the observation group were repaired with sural nerve nutrient vessel flap.The ankle and foot function score,operation time,flap repair(flap length,flap width),complications and postoperative healing were compared between the two groups.Results The ankle and foot function score(92.56±3.80)points of the observation group was higher than(78.22±8.18)points of the control group,and the operation time(1.17±0.08)h was shorter than(2.22±0.12)h of the control group,and the difference was statistically significant(P<0.05).The flap length(19.26±3.21)cm and the flap width(10.47±2.88)cm in the observation group were longer than(12.52±3.28)and(7.82±3.12)cm in the control group,and the differences were statistically significant(P<0.05).The complication rate in the observation group was 3.33%,which was lower than 20.00% in the control group,and the difference was statistically significant(P<0.05).The postoperative healing rate of the observation group was 90.00%,which was higher than 60.00% of the control group,and the difference was statistically significant(P<0.05).Conclusion The sural nerve nutrient vessel flap has better therapeutic effect on the ankle and foot wound surface,and is more valuable than the medial and lateral leg flap,which is worthy of promotion.
