摘  要:目的:探讨分析All-in-one(一站式放疗)方案与常规放疗流程在直肠癌精准放疗中摆位误差的差异性。方法:回顾性分析2021年1月~2021年12月在复旦大学附属肿瘤医院接受放疗的70例直肠癌患者的临床资料,将采用All-in-one的患者纳入观察组,采用常规流程的放疗患者纳入对照组,每组各35例,均采用仰卧位,合适型号的头枕+腿部固定的治疗体位,治疗机选用联影直线加速器uRT-linac 506c。首次治疗前采用FBCT图像引导,后每周一次FBCT图像引导,均采用自动配准方式,对两组数据进行比较分析,分别记录观察组和对照组在左右平移(Lat)、头脚平移(Lng)、腹背平移(Vrt)个方向的平移误差值及在俯仰角(Pitch)、翻滚角(Roll)和偏转角(Yaw)3个方向的旋转误差值,计算其平均值、标准差、系统误差和随机误差,采用独立样本t检验比较数据差异有无统计学意义。结果:观察组和对照组获取的各方向上的摆位误差均值进行独立样本t检验,获得的Lat、Lng、Vrt平移方向上和Pitch、Roll、Yaw旋转方向上的t,P分别为t=-4.787,P=0.000;t=-0.614,P=0.540;t=-4.727,P=0.000;t=-5.142,P=0.000;t=-3.512,P=0.002;t=-2.199,P=0.029。可知,在Lat、Vrt平移方向上和Pitch、Roll、Yaw旋转方向上的差异显著(P<0.05),有统计学意义,而Lng平移方向上的摆位误差数据差异不显著(P>0.05),无统计学意义。结论:All-in-one方案是对常规放疗流程进行了优化并能够减少摆位误差中的系统误差,也可给常规放疗流程进行有效补充。Objective:Compare and analyze the differences between All-in-one radiotherapy solution and conventional radiotherapy procedures in the clinical application of rectal cancer radiotherapy.Methods:The clinical data of 70 patients with rectal cancer who received radiotherapy in Fudan University Shanghai Cancer Center were retrospectively analyzed from January 2021 to December 2021.Patients who received All-in-one radiotherapy were included in observation group,and those who received conventional radiotherapy were included in control group,35 cases in each group,and all patients were in supine position with a suitable head pillow and leg fixation.UIH medical linear accelerator(uRT-linac 506c)was used for radiotherapy treatment.FBCT image guide radiotherapy was used before the first treatment,then FBCT was used once a week,automatic registration was used in all sessions.The two data groups were compared and analyzed,and the translation error(in Lat,Lng and Vrt directions)and the rotation error(in the Pitch,Roll and Yaw directions)were obtained.The average value,standard deviation,systematic error and random error were calculated.Independent samples t-test was used to compare whether the data difference was statistically significant.Results:Independent samples t-test was used to compare the setup error mean values in all directions obtained by observation group and control group,and the t,P values obtained in the translation directions of Lat,Lng,and Vrt and in the rotation directions of Pitch,Roll,and Yaw were t=-4.787,P=0.000;t=-0.614,P=0.540;t=-4.727,P=0.000;t=-5.142,P=0.000;t=-3.512,P=0.002;t=-2.199,P=0.029,respectively.It indicates that the difference in the translation direction of Lat and Vrt and the rotation direction of Pitch,Roll and Yaw is significant(P<0.05),which is statistically significant.In contrast,the setup error in the translation direction of Lng is not significantly different(P>0.05),with no statistical significance.Conclusion:The All-in-one radiotherapy solution optimizes the routine radiotherapy
