螺旋CT在高血压性脑出血合并脑疝中的诊断价值探讨

摘　　要：目的:探讨螺旋CT在高血压性脑出血合并脑疝中的诊断价值。方法:选取2019年3月~2020年10月本院收治的80例高血压性脑出血合并脑疝的患者为研究对象,对其采用螺旋CT加以诊断,分析其脑出血部位及脑疝CT征象。结果:80例患者,其中基底节部位出血人数最多,合计42例,占比52.50%;中线移位距离越大,患者病死率越高,其中10mm以上患者7例,死亡4例,病死率达到57.14%;其中大脑镰下疝50例(62.50%),天幕裂孔疝22例(27.50%),枕骨大孔疝8例(10.00%)。结论:高血压性脑出血合并脑疝患者采用螺旋CT诊断,可清晰显示出其脑出血部位、大小及形态,了解脑组织移位情况,对后续治疗起到借鉴及有价值的参考意义。Objective:To explore the diagnostic value of spiral CT in hypertensive cerebral hemorrhage with cerebral hernia.Methods:From March 2019 to October 2020,80 patients with hypertensive cerebral hemorrhage complicated with cerebral hernia were selected as the research objects,and were diagnosed by spiral CT,the location of cerebral hemorrhage and CT signs of cerebral hernia were analyzed.Results:Of the 80 patients,the basal ganglia had the most bleeding,42 in total,accounting for 52.50%;the greater the midline shift distance,the higher the patient fatality rate.Among them,7 patients were over 10mm,4 patients died,the death rate was 57.14%;50 cases(62.50%)of falciform hernia,22 cases(27.50%)of tentorial hiatus hernia,8 cases(10.00%)of foramen magnum hernia.Conclusion:Spiral CT can clearly show the location,size and shape of cerebral hemorrhage in patients with hypertensive cerebral hemorrhage complicated with cerebral hernia,and understand the displacement of brain tissue.
