摘 要:目的:探究氧气雾化吸入与无创呼吸机治疗慢性阻塞性肺疾病(以下简称慢阻肺)的临床效果。方法:选取2018年10月~2020年4月本院收治的92例慢阻肺患者,根据治疗方式分为无创组(45例)和联合吸入组(47例)。无创组应用无创呼吸机进行机械通气治疗,联合吸入组增加氧驱动雾化吸入治疗,对比两组血气分析与肺功能改善效果。结果:治疗前联合吸入组呼吸频率、PaCO_(2)和PaO_(2)与无创组比较,差异无统计学意义(P>0.05);治疗24h后,联合吸入组呼吸频率、PaCO_(2)均显著低于无创组,而PaO_(2)指标则显著高于无创组,差异均具有统计学意义(P<0.05);治疗72h后,联合吸入组呼吸频率、PaCO_(2)均显著低于无创组,而PaO_(2)指标则显著高于无创组,差异均具有统计学意义(P<0.05);联合吸入组撤机时间显著少于无创组,差异具有统计学意义(P<0.05),治疗10d后,联合吸入组FEV1%、FEV1/FVC均显著高于无创组,差异均具有统计学意义(P<0.05);无创组患者总治疗率为86.7%,低于联合吸入组的95.7%,差异有统计学意义(P<0.05)。结论:无创呼吸机联合氧驱动雾化吸入治疗,可进一步改善氧合效果,提升慢阻肺患者肺功能。Objective:To explore the clinical effect of oxygen atomization inhalation and non-invasive ventilator in the treatment of chronic obstructive pulmonary disease(hereinafter referred to as COPD).Methods:A total of 92 patients with COPD admitted to our hospital from October 2018 to April 2020 were selected and divided into non-invasive group(45 cases)and combined inhalation group(47 cases)according to treatment methods.The non-invasive group was treated with mechanical ventilation with non-invasive ventilator,and the combined inhalation group was treated with atomized inhalation driven by increased oxygen.The blood gas analysis and the improvement effect of lung function were compared between the two groups.Results:Before treatment,there were no significant differences in respiratory rate,PaCO_(2) and PaO_(2) between inhalation group and non-invasive group(P>0.05).After 24h of treatment,the respiratory rate and PaCO_(2) of combined inhalation group were significantly lower than those of non-injury group,while the PaO_(2) indexes were significantly higher than those of non-injury group,with statistical significance(P<0.05).After 72h of treatment,the respiratory rate and PaCO_(2) of combined inhalation group were significantly lower than those of non-injury group,while the PaO_(2) indexes were significantly higher than those of non-injury group,with statistical significance(P<0.05).The withdrawal time of the combined inhalation group was significantly shorter than that of the non-injury group,with statistical significance(P<0.05).After 10 days of treatment,FEV1%and FEV1/FVC of the combined inhalation group were significantly higher than those of the non-injury group,with statistical significance(P<0.05).The total treatment rate of the non-invasive group was 86.7%lower than that of the combined inhalation group(95.7%),the difference was statistically significant,P<0.05.Conclusion:Noninvasive ventilator combined with oxygen-driven atomization inhalation can further improve oxygenation effect and lung function of COPD pa
