动态血糖仪对老年糖尿病胰岛素泵治疗患者的影响

摘　　要：目的:探讨老年糖尿病患者在接受胰岛素泵持续皮下输注胰岛素(CSII)治疗过程中使用动态血糖仪(CGMS)所产生的影响。方法:选择2018年7月~2020年7月本院收治的140例老年糖尿病患者,随机分为对照组70例(采用指尖血糖仪监测血糖)和研究组70例(采用CGMS监测血糖),两组患者均行CSII治疗控制血糖。经医治和评测对比分析两组患者治疗前后血糖、血浆胰岛素和C肽水平差异。结果:治疗前,两组患者CSII血糖指标(空腹血糖、早餐后2h血糖、午餐前血糖等)、胰岛素泵剂量用量及血浆胰岛素与C肽指标组间比较,差异均无统计学意义(P>0.05);治疗后,两组患者以上指标均有明显改善,治疗前后比较差异有统计学意义(P<0.05);治疗后,研究组空腹血糖、午餐前血糖、午餐后2h血糖、睡前血糖指标均显著低于对照组,差异显著(P<0.05)。结论:动态血糖仪在老年糖尿病胰岛素泵治疗患者医治过程中可提高治疗效果,实时反应出患者血糖指标的情况,在更短的时间内控制血糖。Objective:To investigate the effect of continuous subcutaneous insulin infusion(CSII)with insulin pump on the use of CGMS in elderly patients with diabetes mellitus.Methods:A total of 140 elderly diabetic patients admitted to our hospital from July 2018 to July 2020 were selected and randomly divided into a control group of 70(using fingertip blood glucose to monitor blood sugar)and a research group of 70(using CGMS to monitor blood sugar).Both groups were treated with CSII to control blood sugar.The differences in blood glucose,plasma insulin and C-peptide levels before and after treatment were compared between the two groups of patients after treatment and evaluation analysis.Results:Before treatment,there was no significant difference between the two groups in CSII blood glucose indexes(fasting blood glucose,blood glucose 2 hours after breakfast,blood glucose before lunch,etc.),insulin pump dose,plasma insulin and C-peptide indexes(P>0.05);After treatment,the above indexes of the two groups were significantly improved,and the difference was statistically significant before and after treatment(P<0.05);After treatment,fasting blood glucose,blood glucose before lunch,blood glucose 2 hours after lunch and blood glucose before bedtime in the study group were significantly lower than those in the control group(P<0.05).Conclusion:The dynamic blood glucose meter can improve the therapeutic effect in the treatment of elderly patients with diabetes mellitus treated by insulin pump,reflect the situation of blood glucose index in real time,and control blood glucose in a shorter time.
