摘 要:目的研究右美托咪定复合瑞芬太尼的监护麻醉在CT引导下肺癌射频消融术中的应用效果。方法选取本院2019年10月至2020年11月收治的82例经临床或病理证实的非小细胞肺癌(NSCLC)患者,将其随机分为对照组和研究组,各41例。对照组采取丙泊酚复合瑞芬太尼静脉麻醉,研究组采取右美托咪定复合瑞芬太尼的监护麻醉。比较两组的生命体征、复苏情况、不良反应发生情况及满意度评分。结果研究组T_(1)~T_(4)时的MAP均显著高于对照组(P<0.05);两组T_(1)~T_(4)时的HR、RR及SpO_(2)比较,差异无统计学意义(P>0.05)。研究组的清醒时间和离室时间均短于对照组(P<0.05)。研究组的呼吸抑制、低血压发生率均低于对照组(P<0.05)。两组的满意度评分比较,差异无统计学意义(P>0.05)。结论右美托咪定复合瑞芬太尼的监护麻醉应用于CT引导下肺癌射频消融术中,有利于维持患者血流动力学的稳定,提升复苏质量。Objective To study the application effect of monitoring anesthesia of dexmedetomidine combined with remifentanil in CT-guided radiofrequency ablation of lung cancer.Methods Eighty-two patients with clinically or pathologically confirmed non-small cell lung cancer(NSCLC)treated in our hospital from October 2019 to November 2020 were selected and randomly divided into control group and study group,with 41 cases in each group.The control group was given intravenous anesthesia of propofol combined with remifentanil,and the study group was given monitoring anesthesia of dexmedetomidine combined with remifentanil.The vital signs,resuscitation,occurrence of adverse reactions and satisfaction score of the two groups were compared.Results The MAP at T_(1)-T_(4) in the study group was significantly higher than that in the control group(P<0.05);there were no significant differences in HR,RR and SpO_(2) between the two groups at T_(1)-T_(4)(P>0.05).The awake time and out-of-room time in the study group were shorter than those in the control group(P<0.05).The incidences of respiratory depression and hypotension in the study group were lower than those in the control group(P<0.05).There was no significant difference in satisfaction score between the two groups(P>0.05).Conclusion The application of monitoring anesthesia of dexmedetomidine combined with remifentanil in CT-guided radiofrequency ablation of lung cancer is conducive to maintaining the stability of hemodynamics and improving the quality of resuscitation.
