摘　　要：目的结合新版医院评审标准构建临床药师药学监护质量评价体系。方法以医院信息系统为媒介,以药学监护的重点项目为基础,基于关联矩阵法建立药学监护质量评价体系,并通过对比分析我院2020年第三、四季度及2021年第一季度的药学监护数据,评价药学监护质量评价体系的实施效果。结果2020年第三、四季度及2021年第一季度的药学监护数据显示,我院药学会诊的综合指标整体向好,但临床药师用药建议的有效率等方面仍需继续提高。根据药学监护质量评价结果有针对性地改进工作提升了用药教育掌握率、医护患对临床药师工作的满意率和住院患者的医嘱合理率。结论药学监护质量评价体系可以多维度评价临床药师药学监护质量,促进临床药师绩效考核的科学性、规范性,提高临床合理用药水平与药学服务质量。Objective To establish a quality evaluation system for pharmaceutical care of clinical pharmacists in combination with the new version of hospital evaluation standard.Methods Taking the hospital information system as the medium and based on the key projects of pharmaceutical care,the pharmaceutical care quality evaluation system was established based on the incidence matrix method,and the implementation effect of the pharmaceutical care quality evaluation system was evaluated by comparing and analyzing the pharmaceutical care data of our hospital in the third and fourth quarters of 2020 and the first quarter of 2021.Results The pharmaceutical care data in the third and fourth quarters of 2020 and the first quarter of 2021 showed that the comprehensive indexes of pharmaceutical consultation in our hospital were generally good,but the effectiveness of clinical pharmacists'medication suggestions still needed to be improved.Improving the work with a focused goal according to the evaluation results of pharmaceutical care quality improved the mastery rate of medication education,the satisfaction of doctors and patients to the work of clinical pharmacists,and the reasonable rate of medical orders of hospitalized patients.Conclusion The pharmaceutical care quality evaluation system can evaluate the pharmaceutical care quality of clinical pharmacists in multiple dimensions,promote the scientificity and standardization of clinical pharmacists'performance evaluation,and improve the level of clinical rational drug use and the quality of pharmaceutical care.
