摘　　要：目的探讨柴胡疏肝散加减对肝郁型多囊卵巢综合征(PCOS)患者糖脂代谢及性激素水平的影响。方法选取2017年2月至2019年10月就诊于我院的94例肝郁型PCOS患者为研究对象,按治疗方法将其分成对照组和研究组,每组47例。对照组采用常规西医治疗,研究组在此基础上采用柴胡疏肝散加减治疗。比较两组治疗前、后的生殖激素指标、性激素指标、糖脂代谢指标、子宫内膜受容性、中医症状积分。结果治疗后,两组的AMH、T、LH水平均较治疗前显著下降,IHNB、E2水平均较治疗前显著升高,且研究组优于对照组(P<0.05)。治疗后,两组的TC、TG、FINS水平及HbA1c、HOMA-IR均较治疗前显著降低,且研究组低于对照组(P<0.05)。治疗后,两组的子宫内膜厚度均较治疗前显著增加,PI、RI均较治疗前显著降低,且研究组优于对照组(P<0.05)。治疗后,两组的月经紊乱、不孕、肥胖、高雄激素症、脱发积分均较治疗前显著降低,且研究组低于对照组,差异具有统计学意义(P<0.05)。结论柴胡疏肝散加减治疗肝郁型PCOS的效果显著,可通过调节糖脂代谢、性激素,促进子宫内膜受容性恢复,从而增加受孕率、改善症状。Objective To explore the effects of modified Chaihu Shugan powder on glucose and lipid metabolism and sex hormone levels in patients with polycystic ovarian syndrome(PCOS)in hepatic stagnation type.Methods A total of 94 PCOS patients in hepatic stagnation type admitted in our hospital from February 2017 to October 2019 were selected as the study objects and divided into control group and study group according to treatment methods,with 47 cases in each group.The control group was treated with conventional western medicine,and the study group was treated with modified Chaihu Shugan powder on this basis.The reproductive hormone indexes,sex hormone indexes,glucose and lipid metabolism indexes,endometrial tolerance and TCM symptom score were compared between the two groups before and after treatment.Results After treatment,AMH,T and LH levels in both groups significantly decreased compared with those before treatment,while IHNB,E2 levels significantly increased compared with those before treatment,and those in the study group were better than the control group(P<0.05).After treatment,TC,TG,FINS levels,HbA1c and HOMA-IR in both groups significantly decreased compared with those before treatment,and those in the study group were lower than the control group(P<0.05).After treatment,endometrial thickness in both groups significantly increased compared with that before treatment,PI and RI significantly decreased compared with those before treatment,and those in the study group were better than the control group(P<0.05).After treatment,the scores of menstrual disorder,infertility,obesity,hyperandrogenia and hair loss in both groups were significantly lower than those before treatment,and those in the study group were lower than the control group,the differences were statistically significant(P<0.05).Conclusion The modified Chaihu Shugan powder in the treatment of PCOS in hepatic stagnation type has a significant effect,which can promote the recovery of endometrial tolerance by regulating glucose and lipid metabolism and sex
