M型彩色多普勒在评价高血压心脏病左室舒张功能中的作用

摘　　要：目的分析高血压心脏病病患开展M型彩色多普勒超声诊断左心室舒张功能的作用。方法选择本院收治的86例高血压心脏病病患为试验组,2020年1月研究开始,并于2021年3月完成。另选择同时期在本院进行健康体检的86例健康者作为常规组。以彩色多普勒超声诊断仪心脏超声凸阵探头,通过M型彩色多普勒超声以及二维平面超声选择左心室长轴切面和心尖部四腔心切面,对舒张末期室间隔厚度(IVST)、左心室舒张末期内径(LVEDD)、左心室后壁厚度(PWT)、主动脉根部内径(AO)以及左心房内径(LA),持续进行三次测量,并选择平均值作为最终结果。两组均开展M型彩色多普勒超声检查,对比两组左心室结构参数水平、血流相关参数水平;对比主动松弛组、假性正常组以及限制型组病患检测参数。结果常规组与试验组LVDD、左心室收缩末期内径(LVDS)、PWT、IVST水平比较均差异无统计学意义(P>0.05)。常规组与试验组在E峰、A峰、E/A比值、减速时间(DT)、舒张晚期峰值(AV)、收缩期峰值流速(S)、舒张末期流速(D)、S/D比值方面比较均差异无统计学意义(P>0.05);试验组舒张早期充盈血流峰值(EV)以及EV/AV比值均高于常规组,AR低于常规组(P<0.05)。主动松弛组、假性正常组以及限制型组病患E峰、A峰、E/A比值、DT、AV、EV/AV比值、S/D比值、AR水平比较存在明显差异(P<0.05)。结论高血压心脏病病患开展M型彩色多普勒超声检查,能够有效判断病患左心室舒张功能。Objective To analyze the role of M-mode color doppler echocardiography in the diagnosis of left ventricular diastolic function in patients with hypertensive heart disease.Methods A total of 86 patients with hypertensive heart disease were selected as the experimental group,the study began in January 2020 and was completed in March 2020 at 2021.In addition,86 healthy persons who underwent physical examination in our hospital at the same time were selected as the routine group.Color doppler echocardiography was used to diagnose cardiac ultrasonic convex array probe,the left ventricular long-axis section and four-chamber section at the apex of the heart were selected by M-mode color doppler ultrasound and two-dimensional plane ultrasound,the end-diastolic interventricular Septum thickness(IVST),the end-diastolic diameter(LVEDD),the posterior wall thickness(PWT),the AORTIC root diameter(AO)and the left atrial diameter(LA)were measured continuously three times,and the average value was selected as the final result.Both groups were examined by M-mode color doppler ultrasound,the parameters of left ventricular structure and blood flow were compared between the two groups,and the parameters of active relaxation group,pseudo-normal group and restricted group were compared.Results There were no significant differences in LVDD,LVDS,PWT and IVST between the two groups(P>0.05).There were no significant differences in E peak,A peak,E/A ratio,deceleration time(DT),late diastolic peak flow(AV),systolic peak flow(S),end diastolic flow(D)and S/D ratio between the two groups(P>0.05).The peak diastolic filling blood flow(EV)and EV/AV ratio in the experimental group were higher than those in the control group,and AR was lower than that in the control group(P<0.05).There were significant differences in E peak,A peak,E/A ratio,DT,AV,EV/AV ratio,S/D ratio and AR level between active relaxation group,sham normal group and control group(P<0.05).Conclusion M-mode color doppler echocardiography can effectively evaluate left ventricular dias
