摘　　要：目的 评价EarWell耳廓矫形器治疗新生儿耳廓形态畸形的疗效,探讨新生儿耳廓畸形的最佳矫治时间窗,探讨影响矫治时间的因素。方法 将2019年6月至2020年12月门诊诊治的60例(65耳)先天性耳廓形态畸形患儿分为三组,日龄≤7天为第1组共22例(24耳),8-21天为第2组共21例(22耳),22-42天为第3组共17例(19耳)。均采用EarWell耳廓矫形器进行矫治,并比较不同日龄组患儿的矫治时间、有效率和并发症发生率。分析矫治时间与初始矫治年龄、患儿性别及耳廓畸形类型之间的相关性。结果 60例(65耳)患儿经过矫形器的矫治后总体有效率为95.38%(62/65)。1组和2组的有效率均为100%,明显高于3组(84.21%),差异具有统计学意义,P值分别为0.000, 0.002。在矫治时间上1组<2组<3组,差异有统计学意义(P<0.05)。在并发症发生率上1组<2组<3组,差异有统计学意义(P<0.05)。矫治时间与初始矫治年龄及耳廓畸形类型之间有相关性,P值分别为0.000, 0.023,与患儿性别之间无明显相关性(P=0.837>0.05)。结论 EarWell耳廓矫形器对先天性耳廓畸形的患儿矫治效果显著,患儿接受矫治时的年龄及畸形类型是影响矫治时间的因素。矫正年龄越小,疗效越好,矫形器佩戴时间越短,并发症发生率越低。Objective To report efficacy of Earwell auricle orthosis in the treatment of neonatal auricle morphological deformity, and optimal time window for intervention, as well as factors affecting treatment time. Methods Sixty children with congenital auricle malformations(65 ears) treated at the outpatient department from June 2019 to December 2020 were divided into three groups based on age, i.e.. ≤7 days(group 1, n=22, 24 ears), 8-21 days(group 2, n= 21,22 ears) and 22-42 days(group 3 n=17, 19 ears). Earwell auricle orthosis was used to correct the malformation, and treatment time, rate of effectiveness and rate of complications were compared among different age groups. The correlations between time of treatment and the age of initial treatment, sex and type of auricle malformation were analyzed. Results In the 60 cases(65 ears), the overall rate of effectiveness was 95.38%(62/65)(100%, for groups 1 and 2 and84.21% for group 3, P=0.000 and 0.002, respectively). Treatment time was group 1 < group 2 < group 3(P<0.05), with similar trend in incidence of complications among the 3 groups(P<0.05). Duration of treatment was correlated with the age at initial treatment and type of auricle deformity(P=0.000 and 0.023, respectively), but not with gender(P=0.837>0.05). Conclusion Earwell auricle orthosis can be used to effectively treat congenital auricle deformity in children. The age and type of malformation at the time of treatment can affect the time of treatment. The younger the age at the time of treatment, the better the outcome, the shorter the orthotic wear time, and the lower the incidence of complications.
