慢性耳鸣严重程度影响因素分析

摘　　要：目的通过对慢性原发性耳鸣的病史采集、听力学检测、耳鸣心理声学检测、伴发疾病症状采集,探讨哪些因素与慢性耳鸣患者耳鸣严重程度相关。方法2016年4月-2019年12月以耳鸣为第一主诉就诊于秦皇岛市第一医院耳科门诊的病程大于6个月的慢性耳鸣患者,通过填写耳鸣残疾量表(THI)、耳鸣病史采集(性别、年龄、耳鸣病程、侧别、在何种环境下能感知耳鸣)、听力学检测(纯音听阈测定、LDL测试)、耳鸣心理声学检测、可能与耳鸣相关的全身疾病症状(头痛、打鼾、失眠、血管危险因素),应用多因素有序logistic回归进行分析。结果性别、年龄、病程、耳鸣与环境、PTA、耳鸣侧别、匹配频率、匹配响度、残余试验、头痛、血管危险因素、打鼾与慢性耳鸣严重程度之间无相关性。Feldmann曲线类型、听觉过敏、睡眠障碍与慢性耳鸣严重程度之间有相关性。结论严重的慢性主观性耳鸣治疗过程中应关注听觉过敏和睡眠障碍的治疗。Objective To identify factors affecting the severity of chronic tinnitus through analysis of medical history,audiology results,psychoacoustic date and concomitant diseases.Methods Data from patients with chronic tinnitus as the primary complaint for longer than 6 months visiting the Otology Clinic,First Hospital of Qinhuangdao,from April 2016 to December 2019 were collected,including the Tinnitus Handicap Inventory(THI),history of tinnitus(tinnitus course,side and environment where tinnitus was perceived),audiology results(pure tone audiometry[PTA],LDL test),tinnitus related psychoacoustic evaluation,systemic diseases relevant to tinnitus(headaches,snoring,insomnia,vascular risk factors).Multivariate ordered logistic regression was applied for analysis.Results Gender,age,disease course,environment,PTA,tinnitus side,matched tinnitus frequency,matched tinnitus loudness,residual inhibition,headaches,vascular risk factors,snoring were not correlated to the severity of chronic tinnitus.The type of Feldmann Curve,hyperacusis,sleep disorders were found to be correlated to the severity of chronic tinnitus.Conclusion Attention should be paid to the treatment of hyperacusis and sleep disorders in the management of serious chronic subjective tinnitus.
