腹腔镜胆囊切除术中喉罩通气全身麻醉的应用价值

摘　　要：目的分析喉罩通气全身麻醉在腹腔镜胆囊切除术中的应用价值。方法对江苏省东台市中医院行腹腔镜胆囊切除手术的280例患者进行回顾性研究,患者入院时间为2019年5月1日至2021年3月31日,按麻醉方法分为对照组(n=140)和观察组(n=140),对照组行气管插管全身麻醉,观察组行喉罩通气全身麻醉,比较两组胃肠功能恢复情况、不同阶段血流动力学的变化和不良反应的发生情况。结果两组术后腹胀发生率比较,差异无统计学意义(P>0.05),观察组进食流质时间短于对照组,差异有统计学意义(P<0.05);观察组切皮时、CO_(2)气腹后、拔管或拔除喉罩时平均动脉压和心率均低于对照组,差异有统计学意义(P<0.05);观察组不良反应总发生率为5.71%,低于对照组的15.00%,差异有统计学意义(P<0.05)。结论在腹腔镜胆囊切除术中运用喉罩通气全身麻醉,可维持患者血流动力学稳定,且不良反应少,利于患者术后胃肠功能的恢复,有推广价值。Objective To analyze the application value of general anesthesia with laryngeal mask airway(LMA)in laparoscopic cholecystectomy.Methods A total of 280 patients who were admitted for laparoscopic cholecystectomy at Dongtai Hospital of Traditional Chinese Medicine from May 1,2019,to March 31,2021,were retrospectively studied.They were grouped into the contro l group(n=140)and the observation group(n=140)according to the anesthesia methods.The control group received general anesthesia with tracheal intubation and the observation group received general anesthesia with LMA.The recovery of gastrointestinal function,the changes of hemodynamics at different stages and the occurrence of adverse react ions were compared bet ween the two groups.Results There was no statistically significant difference between the two groups in the incidence of postoperative abdominal distension(P>0.05).The time to eat fluids in the observation group was shorter than that in the control group,with s tatistically significant difference(P<0.05).The mean arterial pressure and heart rate at the time of skin incision,after CO_(2) pneumoperitoneum,and at the time of extubation or removal of the laryngeal mask in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).The total incidence of adverse reactions in the observation group was 5.71%,which was lower than 15.00% in the control group,with statistically significant difference(P<0.05).Conclusion During laparoscopic cholecystectomy,the application of general anesthesia with LMA can maintain stable hemodynamics and facilitate the recovery of gastrointestinal function of patients after surgery,with few adverse reactions,which is worthy of promotion.
