摘　　要：目的研究肛周脓肿(PA)伴会阴部坏死性筋膜炎患者行清创术+负压封闭引流技术(VSD)的治疗效果。方法68例PA伴会阴部坏死性筋膜炎患者,随机分为联合组和单纯组,每组34例。单纯组行单纯清创术治疗,联合组采用清创术+VSD术治疗。比较两组手术指标以及并发症发生情况。结果联合组患者体温恢复时间(3.35±0.28)d、创面愈合时间(10.14±2.02)d及住院时间(29.81±4.45)d均短于单纯组的(4.41±0.56)、(13.82±2.45)、(36.32±5.17)d,清创次数(1.95±0.86)次少于单纯组的(4.18±1.34)次,差异具有统计学意义(P<0.05)。联合组并发症发生率5.88%低于单纯组的25.53%,差异具有统计学意义(P<0.05)。结论清创术+VSD术治疗PA合并会阴部坏死性筋膜炎,患者创面愈合快、清创次数少,并发症发生风险低,可提高疗效,缓解患者身心痛苦,安全可靠,值得推荐。Objective To study the effect of debridement combined with vacuum sealing drainage on patients with perianal abscess(PA)and perineal necrotizing fasciitis.Methods A total of 68 patients with PA and perineal necrotizing fasciitis were randomly divided into the combined group and the simple group,with 34 cases in each group.The simple group was treated with debridement alone,and the combined group was treated with debridement and VSD.The surgical indicators and complications were compared between the two groups.Results The body temperature recovery time(3.35±0.28)d,wound healing time(10.14±2.02)d and hospitalization time(29.81±4.45)d in the combined group were all shorter than(4.41±0.56),(13.82±2.45),and(36.32±5.17)d in the simple group,and the number of debridements(1.95±0.86)times was less than(4.18±1.34)times of the simple group.All the differences were statistically significant(P<0.05).The complication rate 5.88% in the combined group was lower than 25.53% in the simple group,and the difference was statistically significant(P<0.05).Conclusion Debridement combined with VSD in the treatment of PA combined with perineal necrotizing fasciitis has the advantages of rapid wound healing,less debridement times and low risk of complications,which can improve the efficacy and alleviate the patients’physical and mental pain.It is safe and reliable,and is worthy of recommendation.
